摘　　要：目的研究原发性肺内孤立性纤维性肿瘤(primary intrapulmonary solitary fibrous tumors,pSFTs)中信号转导和转录激活因子6(signal transducer and activators of transcription 6,STAT6)、配对盒蛋白8(pair box 8,PAX8)的表达情况及临床意义。方法应用免疫组织化学的方法检测19例pSFTs(pSFTs组)及14例肺部其他软组织肿瘤(对照组)中STAT6、PAX8表达情况。比较其与常见标志物CD34、CD99、Bcl-2的敏感度和特异度的差异。分析STAT6和PAX8表达与pSFTs患者临床病理特征的关系。结果STAT6在pSFTs组及对照组肿瘤中的阳性表达率分别为94.74%(18/19)和14.29%(2/14),两者间差异有统计学意义(P<0.05);PAX8在pSFTs组及对照组肿瘤中的阳性表达率分别为31.58%(6/19)和7.14%(1/14),两者间差异无统计学意义(P>0.05)。STAT6在pSFTs中表达的灵敏度和特异度分别为94.74%(18/19)和85.71%(12/14);PAX8在pSFTs中表达的灵敏度和特异度为31.58%(6/19)和92.86%(13/14)。在pSFTs中,CD34、CD99、Bcl-2阳性率分别为78.95%(15/19),68.42%(13/19)和84.21%(16/19);CD34的敏感度和特异度为78.95%(15/19)和85.71%(12/14);CD39的敏感度和特异度为68.42%(13/19)和35.71%(5/14);Bcl-2的敏感度和特异度为84.21%(16/19)和21.43%(3/14)。在pSFTs中,STAT6和PAX8的表达与患者性别、年龄、肿瘤部位、肿瘤直径大小、细胞增殖指数(Ki-67)、细胞核核分裂象等临床特征之间无明显关联(P>0.05)。结论相比传统标志物,STAT6和PAX8表达对于诊断及鉴别诊断肺内SFTs具有明显优势。Objective To study the expression of signal transducer and activators of transcription 6(STAT6)and pair box 8(PAX8)in primary intrapulmonary solitary fibrous tumors and its clinical significance.Methods The immunohistochemical method was used to detect the expression of STAT6 and PAX8 in 19 cases of pSFTs(pSFTs group)and 14 cases of other soft tissue tumors in the lung(control group).The sensitivity and the specificity of common markers CD34,CD99,and Bcl-2 were compared.The relationships of STAT6 and PAX8 expression in pSFTs tissues with clinicopathological features were analyzed.Results The positive expression rates of STAT6 in pSFTs group and control group were 94.74%(18/19)and 14.29%(2/14),respectively,and the difference was statistically significant(P<0.05).The positive expression rates of PAX8 in pSFTs group and control group were 31.58%(6/19)and 7.14%(1/14),respectively,and there was no significant difference(P>0.05).In primary intrapulmonary solitary fibrous tumors,the sensitivity and the specificity of CD34 were 78.95%(15/19)and 85.71%(12/14),respectively.The sensitivity and the specificity of CD99 were 68.42%(13/19)and 35.71%(5/14),respectively.The sensitivity and the specificity of Bcl-2 were 84.21%(16/19)and 21.43%(3/14),respectively.In primary intrapulmonary solitary fibrous tumors,STAT6 and PAX8 expression showed no significant association with patient gender,age,tumor site,tumor diameter,cell proliferation index(Ki-67),nuclear mitosis(P>0.05).Conclusion Compared with the traditional markers,the expression of STAT6 and PAX8 has significant advantages in the diagnosis and differential diagnosis of primary intrapulmonary solitary fibrous tumors.
