老年人听觉和视觉损害与抑郁症状的相关性

摘　　要：目的通过对一项大型全国研究的数据分析,探究老年人听觉和视觉障碍与其抑郁症状的相关性。方法纳入6141例年龄≥60岁的老年人为研究对象,根据是否合并听觉和视觉障碍分为:无障碍组(n=5519)、视觉障碍组(n=195)、听觉障碍组(n=335)和双重障碍组(n=72)。比较一般资料和10条目流调中心用抑郁量表(CESD-10)评分的差异,并分析与CESD-10评分有关的因素。结果四组老年人的年龄、性别构成、受教育年限、婚姻状态和CESD-10评分存在差异(P<0.05)。与无障碍组相比,视觉障碍组、听觉障碍组和双重障碍组的CESD-10评分较高;与听觉障碍组相比,视觉障碍组和双重障碍组的CESD-10评分更高(均P<0.05)。受教育年限低、合并听觉障碍、合并视觉障碍、性别为女性、婚姻状态不佳和年龄低与CESD-10评分升高有关(P<0.05)。结论老年人抑郁症状与其合并听觉和视觉障碍有关,视觉障碍对抑郁症状的影响更明显。Objective To investigate the association between hearing and visual impairments and depressive symptoms in older adults based on a large national study.Methods A total of 6141 elderly patients aged≥60 years were included as study subjects.The patients were divided into barrier-free group(n=5519),visual impairment group(n=195),hearing impairment group(n=335),and double impairment group(n=72).Ten-item center for epidemiologic studies depression scale(CESD-10)scores and demographic data were compared between groups,and the factors associated with CESD-10 scores were analyzed.Results There were significant differences in age,gender composition,education level,marital status,and CESD-10 scores among the four groups(all P<0.05).Compared with barrier-free group,CESD-10 scores were increased in visual impairment group,hearing impairment group,and double impairment group.Compared with hearing impairment group,CESD-10 score was increased in visual impairment group and double impairment group(all P<0.05).Low level of education,complication with hearing impairment,complication with visual impairment,female,poor marital status,and low age were associated with increased CESD-10 score(P<0.05).Conclusion Depressive symptoms in the elderly are associated with hearing and visual impairment,and visual impairment has a more significant effect on depressive symptoms.
