摘 要:目的研究重复经颅磁(rTMS)治疗对慢性精神分裂症患者血清脑源性神经营养因子(BDNF)水平的影响。方法选取2019年3月至2019年10月于武汉市第二精神病医院进行治疗的慢性精神分裂症患者86例。在常规抗精神病用药的基础上,利用SPSS统计学软件将患者随机分成两组:一组患者给予重复经颅磁治疗设为真刺激组(n=43),另一组设为伪刺激组(n=43)。分别在基线时、治疗2周末、治疗4周末检测两组患者的血清BDNF浓度和PANSS量表得分。结果治疗前两组患者PANSS量表评分、血清BDNF浓度差异无统计学意义(P>0.05);治疗4周后,真刺激组的阴性减分值高于伪刺激组,真刺激组的血清BDNF水平高于伪刺激组,差异有统计学意义(P<0.05)。Pearson相关分析显示,治疗4周后真刺激组的PANSS量表阴性减分值与血清BDNF浓度呈正相关(r=0.334,P=0.029)。结论重复性经颅磁持续治疗4周后可有效地改善精神分裂症阴性症状,同时提高血清BDNF的浓度,且BDNF浓度与治疗效果具有相关性。Objective To study the effect of repeated transcranial magnetic stimulation(rTMS)on the level of brain-derived neurotrophic factor(BDNF)in serum in patients with chronic schizophrenia.Methods A total of 86 cases of chronic schizophrenia were selected from March 2019 to October 2019 in Wudong Hospital(Wuhan Second Mental Hospital).Using SPSS statistical software,the patients were randomly divided into two groups:true stimulation group(n=43,repetitive transcranial magnetic stimulation),and false stimulation group(n=43).Serum BDNF concentration and PANSS score were measured in two groups before treatment,at the end of two weeks and four weeks after treatment,respectively.Results There was no significant difference in PANSS score and serum BDNF concentration between the two groups before treatment(P>0.05).At 4 week after treatment,the score of negative reduction in true stimulation group was higher than that in false stimulation group,and the serum BDNF concentration in true stimulation group was higher than that in false stimulation group(P<0.05).There was a positive correlation between negative score of PANSS and serum BDNF concentration at 4 week after treatment(r=0.334,P=0.029).Conclusion Repeated transcranial magnetic therapy for 4 weeks can effectively improve the negative symptoms of schizophrenia,and increase the serum BDNF concentration,and BDNF concentration is related to the efficacy.
