摘 要:目的探讨星形胶质源性蛋白(S100β)、同型半胱氨酸(Hcy)对阿尔茨海默病(AD)诊断及病情评估的价值。方法选取本院2018年6月至2020年12月96例AD患者作为观察组,另选取同期健康体检者48例作为对照组。比较两组血清S100β、Hcy水平,评价血清S100β、Hcy对AD的诊断价值,观察组根据临床痴呆评定量表(CDR)分轻度、中度、重度,对比不同病情患者血清S100β、Hcy、认知功能(MMSE)评分、精神行为(BEHAVE-AD)评分,分析血清S100β、Hcy与MMSE、BEHAVE-AD评分及病情程度间关系。结果观察组血清S100β、Hcy水平高于对照组,差异有统计学意义(P<0.05);血清S100β与Hcy呈正相关(r=0.585,P<0.05);血清S100β、Hcy联合诊断AD的AUC为0.905;随病情加重血清S100β、Hcy、BEHAVE-AD评分升高,MMSE评分降低,血清S100β(r=-0.775、0.736,P<0.001)、Hcy(r=-0.682、0.728,P<0.001)与MMSE评分呈负相关,与BEHAVE-AD评分呈正相关(P<0.05);多元线性回归校正MMSE、BEHAVE-AD评分等其他因素,血清S100β、Hcy仍与AD病情程度相关(P<0.05)。结论血清S100β、Hcy水平变化与AD发生发展成独立显著相关,二者联合检测为临床完善AD诊断机制提供参考。Objective To investigate the value of astrocyte⁃derived protein(S100β)and homocysteine(Hcy)in the diagnosis and evaluation of Alzheimer's disease(AD).Methods A total of 96 AD patients in our hospital from June 2018 to December 2020 were selected as the observation group,and 48 patients with healthy physical examination during the same period were selected as the control group.The serum S100βand Hcy levels were compared between the two groups,and the diagnostic value of serum S100βand Hcy in AD was evaluated.The observation group was divided into mild,moderate,and severe according to the Clinical Dementia Rating(CDR).The serum S100β,Hcy,cognitive function(MMSE)scores,and mental behavior(BEHAVE⁃AD)scores of patients with different conditions were compared,and the relationship between serum S100β,Hcy and MMSE,BEHAVE⁃AD scores and the severity of the disease were analyzed.Results The levels of serum S100βand Hcy in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Serum S100βand Hcy were positively correlated(r=0.585,P<0.05);the AUC of the combined diagnosis of AD by serum S100βand Hcy was 0.905.Serum S100β,Hcy,BEHAVE⁃AD score increased,MMSE score decreased,serum S100β(r=-0.775,0.736,P<0.001),Hcy(r=-0.682,0.728,P<0.001)were negatively correlated with MMSE score,and positively correlated with BEHAVE⁃AD score(P<0.05).Multiple linear regression was used to adjust for other factors such as MMSE,BEHAVE⁃AD score,serum S100βand Hcy were still correlated with the severity of AD(P<0.05).Changes in serum S100βand Hcy levels are independently and significantly correlated with the occurrence and development of AD.The combined detection of the two provides a reference for the clinical improvement of the diagnosis mechanism of AD.
