摘  要:目的研究瞬时感受器电位香草酸受体1(TRPV1)在胃食管反流病(GERD)患者发病中的作用机制。方法选择2019年1—12月就诊于内蒙古科技大学包头医学院第二附属医院消化病研究所胃镜室的患者,采用胃食管反流病问卷(GerdQ)初步入组GERD各亚型患者及正常对照患者共114例,根据临床症状及内镜下食管黏膜表现将入组患者分为反流性食管炎组(RE组)57例、非糜烂性反流病组(NERD组)28例、正常对照组29例,收集其食管黏膜标本,采用免疫组化、Western-blot、RT-PCR等技术检测各组患者食管黏膜TRPV1蛋白阳性染色、蛋白表达量和mRNA表达量。结果免疫组化结果示:在食管鳞状上皮中,RE组和正常对照组、NERD组和正常对照组间TRPV1蛋白阳性表达有统计学差异(P<0.05),其中食管鳞状上皮细胞膜阳性表达显著,RE组阳性、强阳性表达较多,NERD组阳性表达较多,正常对照组弱阳性表达较多;食管鳞状上皮细胞浆阳性表达欠佳,RE组仅可见极个别细胞浆阳性表达,NERD组细胞浆无阳性表达,正常对照组可见部分细胞浆弱阳性表达。Western-blot结果示:与RE组和正常对照组相比,NERD组TRPV1蛋白表达更为显著(P<0.05)。RT-PCR结果示:与NERD组和正常对照组相比,RE组TRPV1 mRNA表达更为显著(P<0.05)。结论TRPV1可能参与RE和NERD患者的食管黏膜损伤、内脏高敏感机制。Objective To study the mechanism of transient receptor potential vanilloid receptor 1(TRPV1)in the pathogenesis of gastroesophageal reflux disease(GERD)patients.Methods Selected patients who were admitted to the gastroscopy room of the Institute of Digestive Diseases,Second Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science and Technology from January to December 2019,and used the Gastroesophageal Reflux Questionnaire(GerdQ)to initially enroll patients with GERD subtypes and A total of 114 normal control patients were divided into reflux esophagitis group(RE group)57 cases and non-erosive reflux disease group(NERD group)28 cases based on clinical symptoms and endoscopic esophageal mucosal manifestations.In 29 cases of normal control group,esophageal mucosal specimens were collected,and immunohistochemistry,Western-blot,RT-PCR and other techniques were used to detect the positive staining of TRPV1 protein,protein expression and mRNA expression of esophageal mucosa in each group.Results The results of immunohistochemistry showed that in the esophageal squamous epithelium,the positive expression of TRPV1 protein was statistically different between the RE group and the normal control group,the NERD group and the normal control group(P<0.05),and the esophageal squamous epithelial cell membrane was positive Significant expression,more positive and strong positive expression in RE group,more positive expression in NERD group,more weak positive expression in normal control group;poor positive expression in esophageal squamous epithelial cytoplasm,and only a few positive cytoplasmic expression in RE group There was no positive expression in the cytoplasm of the NERD group,and weak positive expression in part of the cytoplasm in the normal control group.Western-blot results showed:Compared with RE group and normal control group,TRPV1 protein expression in NERD group was more significant(P<0.05).RT-PCR results showed that compared with the NERD group and the normal control group,the expressio
