多层螺旋CT和MRI在胰腺癌诊断及术前评估中的价值研究

摘　　要：目的探究多层螺旋CT(MSCT)和磁共振成像(MRI)在胰腺癌诊断及术前评估中的价值。方法选择2016年2月至2019年12月期间成都市龙泉驿区第一人民医院收治的胰腺占位患者作为研究对象,所有患者均行MSCT及MRI扫描,以手术及病理检查结果作为金标准,分析两者诊断胰腺癌的价值及术前分期的符合度。结果125患者胰腺癌89例,以病理检查结果作为金标准,MSCT诊断胰腺癌的敏感度、特异度、准确度、阳性预测值、阴性预测值分别为86.52%、72.22%、82.40%、88.51%、68.42%,MRI分别为84.27%、72.22%、80.80%、88.24%、65.00%,两者比较差异均无统计学意义(P>0.05);共59例患者获得外科手术资料,MSCT对Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期评估符合率分别为71.42%、42.86%、76.47%、85.71%,总体符合率为64.41%,MRI对Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期评估符合率分别为78.57%、57.14%、82.35%、100.00%,总体符合率为74.59%;MSCT及MRI评估符合度比较差异无统计学意义(P>0.05);MSCT及MRI评估胰腺癌可切除性符合度为93.75%、93.94%,组间比较差异无统计学意义(P>0.05)。结论MSCT及MRI在胰腺癌诊断及术前评估中均有较高的价值,两者效能相当,临床可酌情选择检查方式。Objective To explore the value of multi-slice spiral CT(MSCT)and magnetic resonance imaging(MRI)in the diagnosis and preoperative evaluation of pancreatic cancer.Methods During the period from February 2016to December 2019,patients with pancreatic space-occupying lesions who were admitted to the First People’s Hospital of Longquanyi District were enrolled as the research objects.All underwent MSCT and MRI scans.Taking the results of surgical and pathological examination as the golden standard,the conformity of the two for diagnosis and preoperative staging of pancreatic cancer.Results Among the 125patients,there were 89cases with pancreatic cancer.Taking pathological examination results as the golden standard,sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MSCT and MRI for diagnosis of pancreatic cancer were(86.52%,72.22%,82.40%,88.51%,68.42%)and(84.27%,72.22%,80.80%,88.24%,65.00%),respectively.And there was no significant difference between the two(P>0.05).There were 59patients undergoing surgical resection.The conformity rates of MSCT for evaluating pancreatic cancer at stageⅠ,Ⅱ,ⅢandⅣwere 71.42%,42.86%,76.47%and 85.71%,respectively.And its overall conformity rate was 64.41%.The conformity rates of MRI for evaluating pancreatic cancer at stageⅠ,Ⅱ,ⅢandⅣwere 78.57%,57.14%,82.35%and 100.00%,respectively.And its overall conformity rate was 74.59%.There was no significant difference in assessment conformity between MSCT and MRI(P>0.05).MSCT and MRI assessment of pancreatic cancer resectability conformity was 93.75%,93.94%,there was no significant difference between the groups(P>0.05).Conclusion Both MSCT and MRI are of high value in the diagnosis and preoperative evaluation of pancreatic cancer.And their efficiency is similar.Clinically,the examination methods can be selected as appropriate.
