摘 要:目的探讨Ⅲ期结肠癌术后辅助化疗敏感性、疗效与亚甲基四氢叶酸还原酶(MTHFR)、谷胱甘肽S转移酶P1(GSTP1)、谷胱甘肽S转移酶M1(GSTM1)基因多态性的关系。方法选择65例行结肠癌切除并接受术后XELOX方案化疗的Ⅲ期结肠癌患者,采血检测MTHFR-rs1801131、GSTP1-rs1695、GSTM1基因多态性,同时回顾性分析患者临床资料及5年生存回访资料,分析不同位点基因多态性与术后辅助化疗敏感性及疗效关系。结果65例患者基因型分布频率均符合Hardy-Weinberg平衡定律。GSTP1-rs1695携带G等位基因,以及GSTM1基因未缺失型患者的化疗敏感率明显较高(P<0.05)。MTHFR-rs1801131携带C等位基因患者的中位无病生存时间和中位总生存期明显较短(P<0.05)。结论MTHFR-rs1801131基因突变可影响Ⅲ期结肠癌患者术后辅助化疗的疗效,GSTP1-rs1695和GSTM1基因突变则与术后辅助化疗敏感性有关。Objective To investigate the relationship between the sensitivity of postoperative adjuvant chemotherapy and curative effect with MTHFR,GSTP1,GSTM1 gene polymorphism in stageⅢcolon cancer.Methods Sixty-five patients with stageⅢcolon cancer undergoing colon cancer excision and receiving postoperative XELOX scheme chemotherapy were selected.The blood was collected for detecting MTHFR-rs1801131,GSTP1-rs1695,GSTM1 gene polymorphism,meanwhile the clinical data and 5-year survival follow up data were retrospectively analyzed,and the relationship between different loci gene polymorphisms with postoperative adjuvant chemotherapy sensitivity and curative effect was analyzed.Results The genotype distribution frequencies of 65 patients were in line with the Hardy-Weinberg equilibrium.GSTP1-rs1695 carried G allele,and the patients with GSTM1 gene non-deletion had higher the chemosensitivity(P<0.05).The median disease-free survival time and median overall survival time in the patients with MTHFR-rs1801131 carrying C allele were significantly shortened(P<0.05).Conclusion MTHFR-rs1801131 gene mutation can affect the curative effect of postoperative adjuvant chemotherapy in the patients with stageⅢcolon cancer.GSTP1-rs1695 and GSTM1 gene mutations are associated with the sensitivity of postoperative adjuvant chemotherapy.
