摘 要:目的比较一体化PET/MR和PET/CT对^(18)F-FDG PET阳性肝转移瘤病灶的检出能力。方法回顾性分析54例经病理及影像随访证实为肝转移瘤的患者。所有患者同一天先后行全身^(18)F-FDG PET/CT显像及上腹部^(18)F-FDG PET/MR局部显像。比较2种检查方法对PET阳性肝转移瘤患者及病灶的检出数量。并根据检出转移瘤病灶的大小进行分组,比较各组中2种检查方法的各自检出数量。结果54例患者中共检出286个PET阳性肝转移瘤病灶。PET/CT检出48例193个阳性病灶,PET/MR检出所有病灶。PET/MR对肝转移瘤患者及病灶的检出数量均高于PET/CT,差异具有统计学意义(均P<0.05)。对于长径<1 cm和1~2 cm的病灶,PET/MR检出数量均高于PET/CT,差异均具有统计学意义(92个vs 15个,77个vs 61个,均P<0.01)。对于长径≥2 cm的病灶,两者检出数量均为117个。结论与PET/CT比较,PET/MR检出到的肝转移瘤数量更多,尤其是在长径<1 cm的肝转移瘤病灶中。Objective To compare the detection ability of PET/MR and PET/CT in ^(18)F-FDG PET-positive liver metastases.Methods Fifty-four patients with liver metastases confirmed by pathology and imaging follow-up underwent ^(18)F-FDG whole-body PET/CT and upper abdomen PET/MR on the same day.The number of PET-positive liver metastasis cases and lesions detected by the two methods were compared.The size of lesions was grouped,and the detection ability of the two methods in each group was compared.Results A total of 286 PET-positive liver metastases were detected in 54 patients.PET/CT detected 193 positive-lesions in 48 patients,PET/MR detected all lesions,and the difference was statistically significant(both P<0.05).For lesions with the maximum diameter(dmax)<1 cm and those with dmax between 1~2 cm,the numbers of lesions detected by PET/MR were both significantly higher than those by PET/CT(92 vs 15,77 vs 61,both P<0.01).For lesions with the dmax≥2 cm,there was no difference in the detection ability of the two methods(117 vs 117).Conclusions Compared with PET/CT,PET/MR detected more liver metastases,especially for lesions with the dmax<1 cm.
