清燥救肺汤加减治疗气阴两虚型非小细胞肺癌慢性咳嗽的效果

摘　　要：目的:研究清燥救肺汤加减治疗气阴两虚型非小细胞肺癌(NSCLC)慢性咳嗽的效果。方法:选取2019年1月-2020年1月的80例气阴两虚型晚期NSCLC患者,根据奇偶数法分为对照组与观察组,每组40例。对照组予吉非替尼治疗,观察组在对照组基础上予清燥救肺汤加减治疗。比较两组疗效、咳嗽症状评分(CSS)、血清炎性指标(IL-6及TNF-α)、外周血T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))及肿瘤进展情况。结果:治疗7、14 d,观察组总有效率均高于对照组,差异均有统计学意义(P<0.05)。治疗前,两组CSS、IL-6、TNF-α比较,差异均无统计学意义(P>0.05);治疗7、14 d,观察组CSS、IL-6、TNF-α均低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组CD3^(+)、CD4^(+)、CD8^(+)比较,差异均无统计学意义(P>0.05);治疗7、14 d,观察组CD3^(+)、CD4^(+)、CD8^(+)均优于对照组,差异均有统计学意义(P<0.05)。观察组肿瘤进展发生率为5.00%,低于对照组的22.50%,差异有统计学意义(P<0.05)。结论:清燥救肺汤加减联合吉非替尼可改善气阴两虚型晚期NSCLC患者的咳嗽症状、炎性反应及免疫状态,有较高的应用价值。Objective:To study the effect of modified Qingzao Jiufei Decoction in the treatment of patients with chronic cough caused by non-small cell lung cancer(NSCLC)of Qi Yin deficiency type.Method:A total of 80 patients with chronic cough caused by NSCLC of qi yin deficiency type from January 2019 to January 2020 were selected,and they were divided into control group and observation group according to odd and even number method,40 cases in each group.The control group was treated with Gefitinib and the observation group was treated with Qingzao Jiufei Decoction on the basis of the control group.The curative effect,cough symptom scores(CSS),serum inflammatory indexes(IL-6 and TNF-α),peripheral blood T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+))and tumor progression were compared between two groups.Result:After 7 and 14 d of treatment,the total effective rates of observation group were higher than those of control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in CSS,IL-6 and TNF-αbetween two groups(P>0.05);after 7 and 14 d of treatment,CSS,IL-6 and TNF-αof observation group were lower than those of control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in CD3^(+),CD4^(+) and CD8^(+) between two groups(P>0.05);after 7 and 14 d of treatment,CD3^(+),CD4^(+) and CD8^(+) of observation group were superior than those of control group,the differences were statistically significant(P<0.05).The incidence of tumor progression of observation group was 5.00%,lower than 22.50%of control group,the difference was statistically significant(P<0.05).Conclusion:Qingzao Jiufei Decoction combined with Gefitinib can improve the cough symptoms,inflammatory response and immune status of advanced NSCLC patients with Qi Yin deficiency type,which has a high application value.
