温阳涤痰消瘀汤联合穴位针刺对急性缺血性脑卒中阳虚痰瘀阻络证患者血液流变学及血浆GMP-140水平的影响

摘　　要：目的:观察温阳涤痰消瘀汤联合穴位针刺对急性缺血性脑卒中(ACI)阳虚痰瘀阻络证患者血液流变学及血浆血小板α颗粒膜蛋白-140(GMP-140)水平的影响。方法:选取2018年11月~2020年11月本院收治的ACI阳虚痰瘀阻络证患者70例,随机数表法分为观察组35例和对照组35例,对照组给予抗血小板聚集、神经保护等常规治疗联合温阳涤痰消瘀汤口服干预,观察组在对照组基础上给予穴位针刺治疗,并观察两组患者的功能评分指标和中医证候积分、血液流变学指标、血清学指标和临床疗效。结果:观察组治疗总有效率为94.29%,高于对照组的68.57%(P<0.05)。治疗后,两组患者的神经功能缺损评分(NIHSS)分值、中医证候积分均减少,正常生活功能评分(BI)增大,并且观察组NIHSS分值、中医证候积分较对照组低,正常生活功能评分(BI)分值较对照组高(P<0.05)。两组患者的全血比粘度、血浆比粘度和纤维蛋白原均低于治疗前,且观察组指标较对照组低(P<0.05)。两组患者的巨噬细胞集落刺激因子(M-CSF)、GMP-140、超敏C反应蛋白(hs-CRP)水平均降低,且观察组低于对照组(P<0.05)。结论:温阳涤痰消瘀汤联合穴位针刺治疗ACI阳虚痰瘀阻络证的临床效果好,并能更好地改善血液流变学指标和血浆GMP-140等血清学指标水平,临床上值得推广。Objective: To explore the effect of Wenyang Ditan Xiaoyu Decoction combined with acupoint acupuncture on hemorheology and level of plasma platelet αgranule membrane protein-140(GMP-140) for patients with acute cerebral infarction(ACI) with Yang-deficiency phlegm stagnation obstruction syndrome. Methods: A total of 70 ACI patients of Yang-deficiency phlegm stagnation obstruction syndrome who were admitted to the hospital from November 2018 to November 2020 were enrolled. According to the random number table method, they were divided into an observation group and a controlled group, 35 cases in each group. The controlled group was given routine treatment(anti-platelet aggregation, neuroprotection) combined with oral Wenyang Ditan Xiaoyu Decoction, while the observation group was given acupoint acupuncture on the basis of the controlled group. The scores of functional scoring indexes and TCM syndromes, hemorheology indexes, serological indexesand clinical curative effect in both groups were observed. Results: The total response rate of treatment in the observation group was higher than that in the controlled group(94.29% vs 68.57%)(P<0.05). After treatment, scores of National Institutes of Health Stroke Scale(NIHSS) and TCM syndromes in both groups were decreased, whilescore of normal life function(BI) was increased. The scores of NIHSS and TCM syndromes in the observation group were lower than those in the controlled group, while BI score was higher than that in the controlled group(P<0.05). After treatment, whole blood viscosity, plasma viscosity and fibrinogen in both groups were decreased, which were lower in the observation group than the controlled group(P<0.05). After treatment, levels of macrophage colony stimulating factor(M-CSF),GMP-140 and high-sensitivity C-reactive protein(hs-CRP) in both groups were decreased, which were lower in the observation group than the controlled group(P<0.05). Conclusion: The clinical curative effect of Wenyang Ditan Xiaoyu Decoction combined with acupoint acupuncture is good
