摘　　要：目的探讨iRoot BP Plus材料结合显微根尖手术治疗难治性根尖周炎的疗效。方法选择2018年6月至2019年6月我院收治的难治性根尖周炎患者96例(101颗患牙),随机分为A组34颗患牙、B组33颗患牙、C组34颗患牙。所有患者均行显微根尖手术治疗,A、B、C组分别使用三氧化矿物凝聚体、银汞合金和iRoot BP Plus进行根尖倒充填。对比3组术前术后的根尖周指数(O-PAI)和术后肿痛率。结果术后6个月、12个月的O-PAI指数A组分别为(1.20±0.51)和(1.09±0.21)、C组分别为(1.05±0.44)和(1.12±0.26),两组均低于B组(P<0.05),A组与C组之间的差异无统计学意义(P>0.05);3组术后6个月、12个月O-PAI指数均低于术前(P<0.05)。A、C组术后1周肿痛率分别为17.65%和11.76%,均低于B组的45.45%(P<0.05),A组与C组之间差异无统计学意义(P>0.05)。结论显微根尖手术使用iRoot BP Plus进行根尖倒充填,其疗效与使用三氧化矿物凝聚体材料相当,值得推广。Objective The study aims to explore the curative effect of iRoot BP Plus material combined with microscopic apical surgery on patients with refractory periapical periodontitis.Methods 96 patients(101 teeth)with refractory periapical periodontitis admitted to our hospital from June 2018 to June 2019 were selected and divided into 32 cases in group A(34 teeth)and B according to the random number table method.There were 32 cases in group(33 teeth)and 32 cases in group C(34 teeth).All patients underwent microscopic apical surgery,group A was treated with mineral oxide aggregates as the backfill material,group B was treated with amalgam as the backfill material,and group C was treated with iRoot BP Plus as the backfill material.The old periapical index(O-PAI)and postoperative swelling and pain rate were compared among the three groups before and after surgery.Results The O-PAI indexes of the 3 groups at 6 months and 12 months after operation were lower than those before the operation(P<0.05),while the O-PAI indexes of group A were(1.20±0.51)at 6 months and 12 months after operation(1.09±0.21),the O-PAI indexes of group C were(1.05±0.44)and(1.12±0.26)at 6 months and 12 months after operation,respectively,which were lower than those of group B(P<0.05),group A and group C There was no significant difference in the O-PAI index between the 6 and 12 months after the operation of the group(P>0.05);the swelling and pain rates of the A and C groups were 17.65%and 11.76%respectively,which were lower than those of the B group.There was no significant difference in the swelling and pain rate of group A and group C at 1 week after operation(P>0.05).Conclusion For patients with refractory periapical periodontitis treated with micro-apical apical surgery,iRoot BP Plus is used as the apical filling material.The curative effect is similar to the use of mineral trioxide aggregate materials,which can reduce postoperative swelling and pain and promote root apex.Bone tissue recovery is worth promoting.
