重性抑郁障碍病人治疗前后中性面孔加工的事件相关电位N170对比研究

摘  要:目的研究重性抑郁障碍病人治疗前后在中性面孔刺激下事件相关电位N170波幅和潜伏期的差异,分析疾病严重程度与N170波幅和潜伏期的相关性。方法选取18~55岁重性抑郁障碍病人28例(病人组)和健康对照者22名(对照组),采集研究对象在呈现中性面孔刺激时的事件相关电位N170与VPP,对病人采用选择性5-羟色胺再摄取抑制剂(SSRIS)类药物治疗,病人组治疗2个月后再次采集中性面孔刺激下的N170与VPP潜伏期与波幅,并采用汉密尔顿抑郁量表(HAMD)评估抑郁症状的严重程度。结果与对照组相比,病人组N170的潜伏期在P7、P8、PO7、PO8、FCZ、CZ电极点上差异有统计学意义(P<0.05),波幅在P7、P8、PO7、PO8、FCZ、CZ电极点上差异有统计学意义(P<0.05)。重性抑郁障碍病人治疗后与治疗前相比,N170潜伏期在P7、PO7、CZ电极点上差异有统计学意义(P<0.05),波幅在P7、P8、PO7、PO8、FCZ、CZ电极点上差异有统计学意义(P<0.05)。病人组HAMD评分与P7、PO7、PO8、CZ、FCZ电极点的N170潜伏期呈正相关(P<0.05),与P7电极点的N170波幅呈正相关(P<0.05),与CZ、FCZ电极点的VPP波幅呈负相关(P<0.05)。结论重性抑郁障碍病人对中性面孔刺激的早期认知加工受损,N170的潜伏期和VPP的波幅可能作为重性抑郁障碍的潜在生物标记物及疗效指标。Objective To investigate the difference of event-related potential(N170)amplitude and latency in patients with major depressive disorder before and after treatment under neutral facial stimuli,and to analyze the correlation between disease severity and N170 amplitude and latency.Methods The event-related potentials N170 and vertex positive potential(VPP)were collected from 28 patients with major depressive depression and 22 healthy controls.Patients with major depressive depression were treated with selective serotonin reuptake inhibitors.After two months of treatment,the latency and amplitude of N170 and VPP were collected in the presence of neutral facial stimuli,and the severity of depression was assessed using the Hamilton Depression Scale(HAMD).Results Compared with the control group,the latent period of N170 in patients with major depressive disorder was statistically significant at the electrode points of P7,P8,PO7,PO8,FCZ and CZ(P<0.05),and the amplitude of N170 was statistically significant at the electrode points of P7,P8,PO7,PO8,FCZ and CZ(P<0.05).Compared with before treatment,the latent period of N170 in patients with major depressive disorder was significantly different at the electrode points of P7,PO7,and CZ(P<0.05),and the amplitude of N170 in patients with major depressive disorder was significantly different at the electrode points of P7,P8,PO7,PO8,FCZ and CZ(P<0.05).HAMD score in patients with major depressive disorder was positively correlated with the latent period of N170 at the electrode points of P7,PO7,PO8,CZ and FCZ(P<0.05),positively correlated with the amplitude of N170 at the electrode points of P7,and negatively correlated with the amplitude of VPP at the electrode points of CZ and FCZ(P<0.05).Conclusion The early cognitive processing of neutral facial stimuli was impaired in patients with major depressive disorder.The latent period of N170 and the amplitude of VPP might be potential biomarkers and therapeutic indicators of major depressive disorder.
