血凝酶联合奥美拉唑治疗胃溃疡伴上消化道出血患者的临床疗效及对细胞因子水平的影响

摘　　要：目的探讨胃溃疡伴上消化道出血患者采用血凝酶联合奥美拉唑治疗的临床疗效及其对患者细胞因子水平的影响。方法选取东营市河口区人民医院收治的200例胃溃疡伴上消化道出血患者作为研究对象,时间为2018年12月至2020年12月,随机分为试验组和对照组,每组100例。两组患者都接受补液、抗感染等常规治疗。对照组患者在常规治疗基础上选择奥美拉唑药物进行治疗,试验组患者在对照组基础上联合血凝酶进行治疗。观察并比较两组患者的治疗效果、治疗前后细胞因子[血管内皮生长因子(VEGF)及表皮生长因子(EGF)]水平、不良反应(头疼、呕血和腹痛等)发生率。结果试验组患者治疗总有效率(96.00%)高于对照组(80.00%)(P<0.05)。治疗前,两组患者VEGF及EGF水平组间比较无差异(P>0.05);治疗后,试验组患者VEGF及EGF水平均明显高于对照组(P<0.05)。试验组患者不良反应发生率(4.00%)明显低于对照组(13.00%)(P<0.05)。结论胃溃疡伴上消化道出血患者通过血凝酶联合奥美拉唑治疗的临床疗效理想,可以在一定程度上使患者的血清细胞因子水平得以提升,减少不良反应的发生。Objective To investigate the clinical effect of hemagglutinin and omeprazole on patients with gastric ulcer and upper gastrointestinal bleeding and its influence on cytokine levels.Methods A total of 200 patients with gastric ulcer and upper gastrointestinal bleeding admitted in Hekou District People's Hospital of Dongying City from December 2018 to December 2020 were selected as the research objects,and they were randomly divided into a experimental group and a control group,with 100 cases in each group.Both groups of patients received routine treatments such as fluid rehydration and anti-infection.Patients in the control group were treated with omeprazole drugs on the basis of conventional treatment,and patients in the experimental group were treated with hemagglutinin on the basis of the control group.Observe and compare the treatment effects of the two groups of patients,the levels of cytokines[vascular endothelial growth factor(VEGF)and epidermal growth factor(EGF)]before and after treatment,and the incidence of adverse reactions(headache,hematemesis,abdominal pain,etc.).Results The total effective rate of treatment in the experimental group was 96.00%,which was higher than 80.00%in the control group(P<0.05).Before treatment,there was no significant difference in the levels of VEGF and EGF between the two groups(P>0.05);after treatment,the levels of VEGF and EGF in the experimental group were significantly higher than in the control group(P<0.05).The incidence of adverse reactions in the experimental group was 4.00%,which was lower than 13.00%in the control group(P<0.05).Conclusion Hemagglutinin combined with omeprazole has an ideal clinical effect in the treatment of patients with gastric ulcer accompanied by upper gastrointestinal bleeding,which can increase the patient's serum cytokine level to a certain extent and reduce the occurrence of adverse reactions.
