床旁血滤对治疗ICU重症急性胰腺炎的临床疗效观察

摘　　要：目的观察床旁血滤对治疗ICU重症急性胰腺炎的临床疗效。方法随机分组,对照组采用常规方法治疗,观察组联合给予床旁血滤,对比治疗效果。结果观察组治疗后CRP(69.98±2.60)mg/L、AMS(114.51±5.41)U/L、Scr(94.32±3.72)μmol/L、BUN(7.95±1.05)mmol/L、腹痛于(40.13±2.03)h消失、住ICU时间为(8.70±1.09)d、有效率92.31%、病死率7.69%。对照组治疗后CRP(123.56±4.98)mg/L、AMS(410.24±5.73)U/L、Scr(135.68±9.22)μmol/L、BUN(20.13±3.88)mmol/L、腹痛于(82.23±3.01)h消失、住ICU时间为(12.65±1.31)d、有效率61.54%、病死率38.46%。两组数据对比,差异显著(P<0.05)。结论给予床旁血滤,可有效促使ICU重症急性胰腺炎患者炎性反应减轻,促进肝肾功能恢复,且能够缩短腹痛消失时间以及住ICU时间,降低患者的病死率,改善预后。Objective To observe the clinical effect of bedside hemofiltration in the treatment of severe acute pancreatitis in ICU.Methods Randomized group,the control group was treated with conventional methods,and the observation group was given combined with bedside hemofiltration to compare the therapeutic effects.Results In the observation group,CRP(69.98±2.60)mg/L,AMS(114.51±5.41)U/L,SCR(94.32±3.72)μmol/L,BUN(7.95±1.05)mmol/L,abdominal pain disappeared in(40.13±2.03)h,the time spent in ICU is(8.70±1.09)d,effective rate 92.31%,mortality 7.69%.In the control group,CRP was(123.56±4.98)mg/L,AMS(410.24±5.73)U/L and SCR was(135.68±9.22)μmol/L,BUN(20.13±3.88)mmol/L after treatment,abdominal pain disappeared in(82.23±3.01)h,the time spent in ICU is(12.65±1.31)d,effective rate 61.54%,mortality 38.46%.The difference between the two groups was significant(P<0.05).Conclusion The application of bedside hemofiltration can effectively reduce the inflammatory response in patients with severe acute pancreatitis in ICU,promote the recovery of liver and kidney function,and shorten the time of disappearance of abdominal pain and the time of ICU,reduce the mortality of patients and improve the prognosis.
