下颌骨牵张成骨术后对上颌骨发育长期随访的研究

摘　　要：目的通过对半侧颜面短小畸形(hemifacial microsomia,HFM)患者术前、术后及随访后头颅正位X线片和三维CT重建影像进行测量,探讨术后长期随访过程中下颌骨牵张成骨术对上颌骨发育的影响。方法自2010年12月至2015年11月,对12例HFM患儿行早期单侧下颌骨牵张成骨术,通过长期随访对患者上颌骨生长发育和骨牵引术后复发的问题进行分析。结果12例HFM患者下颌骨牵张成骨治疗过程顺利,无牵引器脱落,面部对称性均得到较好改善。术后随访3~8年,发现儿童期HMF下颌骨牵张治疗患者上颌骨生长明显,虽有一定程度复发,但面部对称性及咬均得到明显改善。结论采用下颌骨牵张成骨术治疗对HFM患者的上颌骨发育有一定促进作用。建议儿童期HFM患者应尽早进行下颌骨牵张成骨治疗,以获得良好的效果。Objective To explore the maxillary growth after mandibular distraction osteogenesis during the long-term follow-up through measurement of the posterior-anterior cephalometric X-ray films and three-dimensional CT reconstruction images in children with hemifacial microsomia(HFM).Methods From December 2010 to November 2015,12 children with HFM underwent unilateral mandibular distraction osteogenesis at an early age.To analyze the development of maxillary and recurrence after mandibular distraction osteogenesis through a long-term follow-up.Results The process of mandibular distraction osteogenesis in 12 children was successful.No distractor shifting during the treatment.All children got significant improvement in facial asymmetry.During the follow-up from 3 to 8 years,obvious development of maxillary and improvement of facial asymmetry were found,though certain relapse occurred.Conclusion The mandibular distraction osteogenesis can promote the development of maxilla in children with HFM.It is recommended that children with HFM should take mandibular distraction osteogenesis at an early age in order to achieve good outcomes.
