Embosphere微球与PVA颗粒栓塞对子宫肌瘤的治疗效果及对宫颈局部微循环的影响

摘　　要：目的:比较三丙烯明胶(Embosphere)微球与聚乙烯醇(PVA)颗粒栓塞治疗子宫肌瘤的效果及对宫颈局部微循环的影响。方法:抽取2018年1月至2019年2月于洛阳市中心医院行子宫动脉插管栓塞治疗的子宫肌瘤患者70例为研究对象,按随机数字表法分为Embosphere微球组(35例)和PVA颗粒组(35例)。比较两组临床疗效、治疗前后子宫体积、瘤体体积、宫颈局部微循环情况、不良反应发生率。结果:两组治疗后3、6个月,临床治疗有效率、子宫体积、瘤体体积比较,P>0.05。Embosphere微球组治疗后宫颈局部微血管管径、毛细血管管径小于PVA颗粒组(P<0.05);Embosphere微球组治疗后血流灌注量少于PVA颗粒组(P<0.05);Embosphere微球组不良反应发生率(11.43%)低于PVA颗粒组(31.43%),P<0.05。结论:Embosphere微球与PVA颗粒栓塞治疗子宫肌瘤效果相当,但Embosphere微球可以缩小患者宫颈局部微血管、毛细血管管径,不良反应发生率低。Objective To explore curative effect of trisacryl gelatin(Embosphere)microspheres and polyvinyl alcohol(PVA)particles on uterine fibroids and their influences on local cervical microcirculation.Methods A total of 70 patients with uterine fibroids who underwent uterus artery catheterization embolization in Luoyang Central Hospital from January 2018 to February 2019 were enrolled.They were divided into Embosphere microspheres group(35 cases)and PVA particles group(35 cases)by random number table method.The clinical curative effect,uterus volume,tumor volume,local cervical microcirculation and incidence of adverse reactions before and after treatment were compared between the two groups.Results At 3 months and 6 months after treatment,the clinical effective rate,uterine volume and tumor volume were compared between the two groups,P>0.05.After treatment,diameter of cervical local microvascular and blood capillaryin Embosphere microspheres group was smaller than that in PVA particles group(P<0.05),the blood perfusion volume of Embosphere microspheres group was less than that of PVA particles group(P<0.05),and incidence of adverse reactions was lower than that in PVA particles group(11.43%vs.31.43%),P<0.05.Conclusions The curative effect of embosphere microspheres and PVA particles embolization is comparable on uterine fibroids.However,the former can reduce diameter of cervical local microvascular and blood capillary,with low incidence of adverse reactions.
