经脐单孔腹腔镜应用于卵巢巨大囊肿剥除术的安全性和可行性研究

摘　　要：目的探讨经脐单孔腹腔镜行囊肿剥除术治疗卵巢巨大囊肿的安全性及可行性。方法回顾性分析我院收治的57例卵巢巨大囊肿(直径≥10 cm)患者的临床资料,根据手术方式将其分为观察组(25例)和对照组(32例)。观察组行经脐单孔腹腔镜囊肿剥除术,对照组行传统腹腔镜囊肿剥除术。记录2组患者的手术相关指标、术后并发症情况、术后疼痛情况及术后美观满意度,比较2组患者术前及术后1个月的卵巢功能指标,包括促黄体生成素(LH)、促卵细胞生成激素(FSH)、雌二醇(E2)。结果观察组手术时间、术中出血量、囊液外漏率、术后排气时间及术后住院时间等手术相关指标均优于对照组,观察组术后并发症总发生率及术后24 h VAS评分均较对照组低,术后1个月美观满意度较对照组高,差异均有统计学意义(P<0.05)。2组术后1个月LH、FSH、E2水平与术前比较,差异均有统计学意义(P<0.05);观察组术后1个月LH、FSH水平较对照组低,E2水平较对照组高,差异均有统计学意义(P<0.05)。结论经脐单孔腹腔镜囊肿剥除术治疗卵巢巨大囊肿安全可行,且手术时间短,术中出血量少,囊液外漏率低,术后恢复更快,术后并发症少,术后疼痛感轻,美观度更高,对患者卵巢功能损伤较小。Objective To investigate the safety and feasibility of transumbilical single-port laparoscopic cystectomy in the treatment of giant ovarian cyst.Methods The clinical data of 57 patients with giant ovarian cyst(diameter≥10 cm)admitted to our hospital were retrospectively analyzed.According to the surgical approach,they were divided into the observation group(25 cases)and the control group(32 cases).The observation group received transumbilical single-port laparoscopic cystectomy,and the control group received traditional laparoscopic cystectomy.The operation related indexes,postoperative complications,postoperative pain and postoperative aesthetic satisfaction of the two groups were recorded.The ovarian function indexes,including luteinizing hormone(LH),follicular stimulating hormone(FSH)and estradiol(E2),were compared between the two groups before and 1 month after the operation.Results The operation time,intraoperative blood loss,cystic fluid leakage rate,postoperative anal exhaust time and postoperative hospital stay of the observation group were all better than those of the control group,the total incidence of postoperative complications and VAS score 24 hours after the operation in the observation group were lower than those in the control group,and the aesthetic satisfaction 1 month after the operation in the observation group was higher than that of the control group,and the differences were all statistically significant(P<0.05).There were statistically significant differences in the levels of LH,FSH and E2 in the two groups 1 month after the operation were higher than those before the operation(P<0.05).The levels of LH and FSH in the observation group were lower than those in the control group 1 month after the operation,the level of E2 was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Transumbilical single-port laparoscopic cystectomy is safe and feasible for gaint ovarian cyst,with shorter operation time,less intraoperative blood loss,lower
