1例肺泡蛋白沉积症患儿行全肺灌洗治疗的护理

摘　　要：总结1例因IARS基因突变导致肺泡蛋白沉积症患儿行全肺灌洗治疗围手术期的护理经验。护理要点包括:术前评估患儿对于灌洗治疗的耐受情况,完善术前准备,实施医疗游戏辅导,减少患儿哭吵及氧消耗;术中医护配合完成灌洗治疗,监测患儿血氧饱和度变化;术后进行气道管理、控制感染及监测相关并发症。经过2次入院行全肺灌洗治疗,患儿呼吸困难症状好转,血氧饱和度保持稳定。The primary objective of this study is to share the perioperative nursing experience of whole lung lavage for a pulmonary alveolar proteinosis(PAP)child with IRAS gene mutation.The key points of perioperative nursing measurements comprise evaluating children’tolerance of lavage procedure,improving preoperative preparation and applying play therapy to alleviate children’s distress and oxygen uptake before the operation;enhancing the cooperation with doctors and monitoring the oxygen saturation during the operation;enhancing the airway care,preventing infections and monitoring complications after the operation.The symptoms of dyspnea were greatly improved and the blood oxygen saturation reached to expectations after twice of bronchoalveolar lavage.
