摘 要:目的分析运动锻炼对维持性血液透析患者心肺功能和心理状态的改善作用。方法选取2015年3月至2017年12月四川大学华西医院收治的99例维持性血液透析患者为研究对象,按照随机数字表法将入选患者分为观察组(50例)和对照组(49例),两组患者均接受血液透析和基础治疗,观察组患者在此基础上接受运动锻炼干预,比较两组患者治疗前和治疗12周后心肺功能、心理状态、生活质量和治疗期间的安全性。结果治疗后,观察组患者最大摄氧量、代谢当量、健康调查量表36评分均显著高于本组治疗前和同期对照组(均P<0.05),负荷运动时间均显著长于本组治疗前和同期对照组(均P<0.05),焦虑自评量表和抑郁自评量表评分均显著低于本组治疗前和同期对照组(均P<0.05)。两组患者治疗期间均未发生心绞痛、猝死、心律失常、心力衰竭加重等不良事件。结论运动锻炼能够有效提升维持性血液透析患者的心肺功能,促进其心理状态和生活质量的改善。Objective To analyze the clinical improvement effect of exercise training on cardiopulmonary function and psychological status in patients with maintenance hemodialysis.Method From March 2015 to December 2017,99 cases of patients with maintenance hemodialysis admitted to West China Hospital,Sichuan University were selected for study,and they were divided into observation group(n=50)and control group(n=49)according to random number table method.All patients were given hemodialysis and basic treatment,and observation group patients were given exercise training intervention on this basis.The cardiopulmonary function,psychological status,quality of life before treatment and 12 weeks after treatment and safety during treatment of the two groups were compared.Result After treatment,the maximal oxygen uptake,metabolic equivalent,and 36-item short form health survey score of patients in observation group were significantly higher than those before treatment and control group in the same period(all P<0.05),the duration of load exercise was significantly longer than that before treatment and control group in the same period(all P<0.05),the self-rating anxiety scale and self-rating depression scale scores were significantly lower than those before treatment and control group in the same period(all P<0.05).No adverse events such as angina pectoris,sudden death,arrhythmia,and aggravation of heart failure occurred in the two groups during treatment.Conclusion Exercise training can effectively improve cardiopulmonary function and promote the improvement of psychological status and quality of life of patients with maintenance hemodialysis.
