加减清肝化湿活血汤联合多烯磷脂酰胆碱在酒精性脂肪肝患者中的应用

摘　　要：目的探讨加减清肝化湿活血汤联合多烯磷脂酰胆碱在酒精性脂肪肝患者中的应用。方法选取2017年11月—2019年11月湖北省荆州市胸科医院收治的200例酒精性脂肪肝患者作为研究对象,采用随机数字表法将其分为研究组与对照组,每组各100例。对照组患者接受多烯磷脂酰胆碱治疗,研究组患者在对照组的基础上加用加减清肝化湿活血汤。比较两组患者的疗效、肝功能指标[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)以及γ-谷氨酰胺转肽酶(GGT)]、血脂水平[总胆固醇(TC)、三酰甘油(TG)]、血清纤维蛋白原样蛋白1(FLP1)、Ⅲ型前胶原肽(PⅢP)水平。结果研究组患者的总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组患者的血清ALT、AST、GGT、TC、TG水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组患者的血清FLP1高于对照组,PⅢP低于对照组,差异有统计学意义(P<0.05)。结论加减清肝化湿活血汤联合多烯磷脂酰胆碱治疗酒精性脂肪肝效果良好,可明显改善患者血脂代谢和肝功能,减轻肝纤维化程度。Objective To explore the application of modified Qinggan Huashi Huoxue Decoction combined with Polyene Phosphatidyl Choline in patients with alcoholic fatty liver.Methods A total of 200 patients with alcoholic fatty liver who were diagnosed and treated in Chest Hospital of Jingzhou City,Hubei Province from November 2017 to November 2019 were selected as the study objects.The patients were divided into the study group and the control group by random number table method,with 100 cases in each group.The control group was treated with Polyene Phosphatidyl Choline,and the study group was treated with modified Qinggan Huashi Huoxue Decoction on the basis of the control group.Therapeutic effect,liver function indexes(alanine aminotransferase[ALT],aspartate aminotransferase[AST]andγ-glutamine transpeptidase[GGT]),blood lipid level(total cholesterol[TC],triglyceride[TG]),serum fibrinogen protein 1(FLP1),typeⅢprocollagen peptide(PⅢP)level were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of ALT,AST,GGT,TC and TG in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the FLP1 in the study group was higher than that in the control group,and the PⅢP was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion Modified Qinggan Huashi Huoxue Decoction combined with Polyene Phosphatidyl Choline is effective in the treatment of alcoholic fatty liver.It can obviously improve the metabolism of blood lipid and liver function,and reduce the degree of liver fibrosis.
