彩色多普勒超声对囊性肾癌的诊断价值分析

摘　　要：目的分析彩色多普勒超声对囊性肾癌(CRCC)的诊断价值。方法收集90例肾脏囊性病变患者的病历资料,经手术病理确诊囊性肾瘤(CN)患者24例,CRCC患者66例。以手术病理结果为金标准,计算彩色多普勒超声、超声造影(CEUS)及二者联合应用对CRCC的诊断效能指标(灵敏度、特异度、准确度、阳性预测值、阴性预测值),以受试者工作特征(ROC)曲线分析诊断价值,对比CRCC与CN的彩色多普勒超声、CEUS特征。结果彩色多普勒超声与CEUS联合诊断CRCC的各项诊断效能指标均最高,与病理检查结果具有较高一致性,CEUS与病理检查结果具有较高一致性,彩色多普勒超声与病理检查结果一致性较低。彩色多普勒超声与CEUS联合应用对CRCC的诊断价值最高,CEUS对CRCC具有较高诊断价值,彩色多普勒超声对CRCC诊断价值较低。CRCC患者探及结节、探及血流信号比例均明显高于CN患者,差异均有统计学意义(P﹤0.01)。CRCC患者与CN患者囊壁与分隔厚度、增强达峰强度、增强实性结节、Bosniak分级、造影增强模式比较,差异均有统计学意义(P﹤0.01)。结论彩色多普勒超声联合CEUS对CRCC具有较高诊断价值,彩色多普勒超声更适宜应用于筛查与初步诊断中,疑似CRCC可通过CEUS提高诊断准确率。Objective To analyze the diagnostic value of color Doppler ultrasound in cystic renal carcinoma(CRCC).Method Clinical data of 90 patients with renal cystic lesions were collected,24 patients with cystic nephroma(CN)and66 patients with CRCC were confirmed by operation and pathology.Taking the surgical pathological results as the gold standard,the diagnostic efficacy indexes(sensitivity,specificity,accuracy,positive predictive value and negative predictive value)of color Doppler ultrasound,contrast-enhanced ultrasound(CEUS)and their combined application for CRCC were calculated.The diagnostic value was analyzed by the receiver operating characteristic(ROC)curve,and the characteristics of color Doppler ultrasound and CEUS of CRCC and CN were compared.Result The diagnostic efficacy indexes of color Doppler ultrasound combined with CEUS in the diagnosis of CRCC were the highest,which was highly consistent with the results of pathological examination.CEUS was highly consistent with the results of pathological examination,and the consistency between color Doppler ultrasound and pathological examination was lower.The combination of color Doppler ultrasound and CEUS had the highest diagnostic value for CRCC,CEUS had higher diagnostic value for CRCC,and color Doppler ultrasound had lower diagnostic value for CRCC.The proportion of detecting nodules and blood flow signals in CRCC patients were significantly higher than those in CN patients(P<0.01).There were significant differences in capsule wall and septum thickness,enhancement peak intensity,enhancement solid nodules,Bosniak grade and contrast enhancement mode between CRCC and CN patients(P<0.01).Conclusion Color Doppler ultrasound combined with CEUS has high diagnostic value for CRCC,color Doppler ultrasound is more suitable for screening and preliminary diagnosis,the diagnostic accuracy of suspected CRCC can be improved by CEUS.
