囊腔型肺癌的CT特征动态变化及病理对照分析

摘　　要：目的探讨囊腔型肺癌的CT特征和动态演变过程,提高对本病的认知和诊断准确性。资料与方法回顾性分析经病理证实的囊腔型肺癌22例,根据病灶的形态学特点分为4种类型,并分析囊腔型肺癌的CT表现,评价病灶的动态变化。结果22例中,肺腺癌20例,鳞状细胞癌2例。16例(73%)囊腔型肺癌病灶发生于两肺中上叶,12例(55%)发生于肺胸膜下区。术前末次囊腔型肺癌的主要CT表现为多囊17例、囊腔内壁不光整15例、囊内分隔18例。随着肿瘤进展,18例病灶实性成分增多;囊腔缩小7例,囊腔增大8例,未发生明显变化1例;2例囊腔型肺癌由磨玻璃密度结节演变而来;2例病灶进展为厚壁空洞型肺癌;1例囊腔型肺癌发生在正常肺组织基础上。结论囊腔内壁毛糙、囊内分隔及壁结节是有助于囊腔型肺癌诊断的特征性征象,观察动态演变有助于提高术前诊断准确性。Purpose To compare the CT serial findings and pathological features in patient with cystic lung cancer,improving the understanding and diagnostic accuracy of cystic lung cancer in clinical practice.Materials and Methods A total of 22 cases pathologically confirmed cystic lung cancer were retrospectively analyzed.Based on the morphologic patterns,all lesions were divided into four types,evaluating CT findings and the dynamic features.Results Of the 22 patients,there were 20 cases with lung adenocarcinoma and 2 cases with squamous cell carcinoma.A total of 16 cases(73%)with cystic lung cancer were located in the middle and upper lobes of bilateral lungs,and 12 cases(55%)were located in the subpleural area of the lung.The preoperative CT findings of cystic lung cancer were mainly presented with polycystic(15 cases),uneven inner wall of the cyst(15 cases),and intra-cystic separation(16 cases),etc.As the lesion progressed,the increased solid components of the lesions were showed in 18 cases,and there were 7 cases with decreased cyst cavity,8 cases with increased cyst cavity,and 1 case without significant change.In addition,2 cases with cystic lung cancer were evolved from ground-glass density nodules,2 cases were progressed to thick-walled cavity lung cancer,and 1 case with cystic lung cancer was occurred in the normal lung tissue.Conclusion The CT findings of cystic lung cancer are with characteristic and specific features,especially with the dynamic observation.
