摘 要:目的:探讨口腔溃疡防护剂对放射性食管炎的影响。方法:选取2017年12月至2018年10月我院收治的明确诊断的食管癌患者48例,在同步放化疗期间,出现1级及以上食管炎时随机接受口腔溃疡防护剂(观察组)及通关液(对照组,包括利多卡因注射液+地塞米松注射液注射液+庆大霉素注射液+维生素B12)治疗。比较两组患者放射性食管炎的发生率,疼痛评分,治疗有效率;观察血液中白细胞、淋巴细胞、中性粒细胞、血红蛋白、白蛋白的变化情况;观察C-反应蛋白(CRP)、白细胞介素-6(IL-6)及降钙素原(PCT)的变化情况。结果:两组患者均未发生Ⅳ级放射性食管炎;观察组放射性食管炎的发生率低于对照组(P<0.05);两组患者治疗后NRS评分差异有统计学意义(P<0.05);观察组疼痛缓解有效率显著高于对照组(P<0.05);两组治疗前后白细胞、中性粒细胞、白蛋白、血小板变化无统计学差异(P>0.05);治疗后两组CRP、IL-6、PCT差异有统计学意义(P<0.05)。结论:口腔溃疡防护剂可有效防治食管癌放疗中放射性食管炎。Objective:To explore the effectiveness of oral ulcer protective agent in reducing radiation esophagitis during radiotherapy of esophageal cancer.Methods:A total of 48 patients with well-diagnosed esophageal cancer recruitted the second ward of the radiotherapy de-partment of our hospital from December 2017 to October 2018,and randomly received oral ulcer protective agents when they developed esophagitis of grade 1 or above during concurrent radiotherapy and chemotherapy(observation group)and Tongguan solution(control group,including lidocaine injection,dexamethasone injection,gentamicin injection,vitamin B12)treatment.To compare the incidence of esophagitis,pain score,and effective treatment efficiency between the two groups of patients.And to observe the changes of white blood cells,neutrophils,lymphocytes,hemoglobin,and albumin in the blood.And to observe C-reactive protein(CRP)and decrease Changes in procalcin(PCT)and interleukin-6(IL-6).Results:GradeⅣradiation esophagitis did not occur in both groups.The incidence of radiation esophagitis in the obser-vation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The two groups have statistically significant differences in NRS scores after treatment(P<0.05).The effective rate of pain relief in the observation group is significantly higher than that of the control group,The difference was statistically significant(P<0.05).There was no significant difference in the changes of white blood cells,neutrophils,albumin,and platelets between the two groups before and after treatment(P>0.05),CRP,L-6,and PCT showed signifi-cant differences after treatment,and before treatment,the difference was not statistically significant(P<0.05).Conclusion:The oral ulcer pro-tective agent can effectively prevent and treat radiation esophagitis during radiotherapy of esophageal cancer.
