摘　　要：目的探讨腹腔镜全直肠系膜切除术对低位直肠癌患者术后胃肠动力恢复及肛门功能的影响。方法选取2018年6月至2020年6月本院收治的低位直肠癌患者60例,采用随机数字表法分为两组,各30例。对照组行传统开腹术,观察组行腹腔镜全直肠系膜切除术。比较两组胃肠动力指标[胃动素(MOT)、胃泌素(GAS)、碱性成纤维细胞生长因子(bFGF)]及肛门功能[直肠最大耐受量(RMTV)、肛管静息压(ARP)、肛管最大收缩压(AMSP)]。结果治疗后,观察组MOT、GAS、RMTV高于对照组,差异均有统计学意义(P<0.05)。治疗前,两组肛门功能指标比较差异无统计学意义;治疗后,两组ARP、AMSP比较差异无统计学意义,观察组RMTV高于对照组,差异有统计学意义(P<0.05)。结论腹腔镜全直肠系膜切除术治疗低位直肠癌患者效果显著,对患者胃肠动力指标、肛门功能影响较小,利于改善患者预后。Objective To investigate the effect of laparoscopic total mesentery resection on postoperative gastrointestinal motility and anal function in patients with low rectal cancer after operation.Methods 60 patients with low rectal cancer admitted to our hospital from June 2018 to June 2020 were selected and they were divided into two groups by random number table method,with 30 cases in each group.The control group underwent traditional laparotomy,and the observation group underwent laparoscopic total mesentery resection.The gastrointestinal motility indexs[motilin(MOT),gastrin(GAS),basic fibroblast growth factor(bFGF)],and anal function[maximum tolerable rectal dose(RMTV),anal resting pressure(ARP),and anal maximum systolic pressure(AMSP)]were compared between the two groups.Results After treatment,MOT,GAS,RMTV in the observation group were higher than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no statistical difference in anal function between the two groups;after treatment,there was no significant difference between the two groups in ARP and AMSP,RMTV in observation group was higher than the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic total mesentery resection is effective in the treatment of patients with low rectal cancer,and has little effect on gastrointestinal motility indexs and anal function,which is beneficial to improve the prognosis of patients.
