摘 要:目的:探讨子宫内膜癌(EC)组织中类固醇生成因子-1(SF-1)启动子甲基化状态、基因表达及其临床意义。方法:应用亚硫酸氢测序、蛋白免疫印迹法、免疫组化检测重庆医科大学附属第一医院2013年1月至2014年8月收治的60例EC患者的术后标本、30例癌旁组织和20例正常子宫内膜组织中SF-1启动子甲基化状态和基因的表达,分析SF-1蛋白表达与肿瘤临床病理特征的关系。结果:(1)与癌旁组织和正常子宫内膜组织相比,EC组织中SF-1蛋白表达显著升高(P<0.05)。(2)SF-1蛋白的表达在不同的组织分级和病理类型的比较,差异有统计学意义(P<0.05),在不同的年龄、手术病理分期、肌层浸润深度和淋巴结转移间比较,差异无统计学意义(P>0.05)。(3)DNA甲基转移酶(DNMT)3a和DNMT3b在EC组织中的表达显著低于癌旁组织(P<0.05)。(4)EC组织中SF-1基因启动子区甲基化胞嘧啶百分比显著低于癌旁组织(8.2%vs 40.9%,P<0.05)。结论:EC组织中SF-1启动子呈低甲基化,SF-1蛋白表达显著增高,SF-1蛋白异常高表达与患者的临床病理特征有关,可能促进EC的发生、发展。Objective:To investigate the methylation status and gene expression of SF-1 in endometrial carcinoma(EC)tissue and explore its clinical significance.Methods:Bisulfite sequencing,western blotting,and immunohistochemistry were used to detectthe methylation status and protein expression of SF-1 in 60 cases of EC tissue,30 cases of paracancerous tissue and 20 cases of normal endometrial tissue which were collected in The First Affiliated Hospital of Chongqing Medical University between January 2013 and August 2014.The correlation between SF-1 gene expression and clinic opathological factors in EC was analyzed byχ2 test and t test.Results:(1)Compared with paracancerous tissue and normal endometrial tissue,the SF-1 gene promoter in EC tissue was significantly hypomethylated(P<0.05).(2)The expression of SF-1 protein in EC tissue was expressed significantly higher than that in normal endometrial tissue and paracancerous tissue(P<0.05).SF-1 expression was statistically correlated with pathological grade and pathological type(P<0.05),but there was no statistical significance with age,stage,depth of muscular invasion and lymph node metastasis(P>0.05).(3)As the DNA methyltransferases,the expression of DNMT3a and DNMT3b in endometrial cancer tissue was significantly lower than that in paracancerous tissue(P<0.05).(4)The percentage of methylcytosine in the promoter region of the SF-1 gene in endometrial cancer tissue was significantly lower than that in paracancerous tissue(8.2%vs 40.9%,P<0.05).Conclusions:In endometrial carcinoma,the SF-1 manifestshypomethylated and the expression of SF-1 is significantly increased.It indicates that SF-1 may be strictly related to the development and progression of endometrial cancer and abnormally SF-1 high expression is associated with poor prognosis.
