肿瘤坏死因子-α在胃癌组织中的表达及其临床意义

摘　　要：目的探讨在胃癌组织中肿瘤坏死因子-α(TNF-α)的表达及其临床意义。方法抽取自苏州大学附属第一医院于2018年12月至2020年12月,收治的胃癌患者46例进行观察研究,行手术切除中选择胃癌组织标本,运用酶联免疫吸附试验(ELISA)和免疫组织化学法测定包括46例胃癌原发灶、癌旁组织与25例胃癌患者正常胃黏膜组织中TNF-α的表达。数据采用单因素方差分析、LSD-t检验和χ^(2)检验。结果癌旁组织组(261.21±21.16)μg/L与胃癌原发灶组TNF-α表达水平(172.57±15.09)μg/L高于正常组织组(35.21±4.16)μg/L,差异有统计学意义(F=24.125,P<0.05)。Ⅱ期(211.42±20.65)μg/L、Ⅲ期TNF-α表达水平(305.26±28.46)μg/L高于病理分期Ⅰ期(182.35±17.35)μg/L,差异有统计学意义(F=18.337,P<0.05);N1期(236.57±22.49)μg/L、N2期(274.24±25.21)μg/L、N3期TNF-α表达水平(361.45±33.35)μg/L高于N0期(185.21±17.36)μg/L,差异有统计学意义(F=16.576,P<0.05)。T2期(41.67%)、T3期(52.38%)与T4期胃癌组织TNF-α阳性表达率(77.78%)远高于T1期(25.00%,χ^(2)=11.547,P<0.05);T3、T4期胃癌组织TNF-α阳性表达率(52.38%、77.78%)远高于T2期(41.67%),差异有统计学意义(χ^(2)=9.252,P<0.05)。低分化组(50.00%)、中分化组(50.00%)、高分化组TNF-α阳性表达率(55.56%)差异无统计学意义(χ^(2)=0.858,P>0.05)。结论TNF-α高表达与胃癌的T分期、分化程度、淋巴结转移密切相关,可参与胃癌病症的发生、进展。Objective To investigate the expression of tumor necrosis factor-α(TNF-α)in gastric cancer and its clinical significance.Methods Gastric cancer tissue samples were collected from surgical resections of 46 gastric cancer patients obtained from the First Affiliated Hospital of Soochow University between December 2018 and December 2020.Enzyme linked immunosorbent assay(ELISA)and immunohistochemistry were used to measure TNF-αexpression in 46 cases of gastric cancer primary lesions and adjacent tissues,and 25 cases of normal gastric mucosa tissues.Data were analyzed by SPSS23.0,and one-way ANOVA,LSD-t test,and chi square test were performed.Results The expression levels of TNF-αin paracancerous tissue group and primary gastric cancer group were(261.21±21.16)and(172.57±15.09)μg/L respectively,which were significantly higher than those in normal gastric mucosa group[(35.21±4.16)μg/L,F=24.125,P<0.05].The expression level of TNF-αin gastric cancer patients with stageⅡandⅢwas(211.42±20.65)and(305.26±28.46)μg/L respectively,which was significantly higher than that in patients with pathological stageⅠ[(182.35±17.35)μg/L,F=18.337,P<0.05].The expression level of TNF-αin gastric cancer patients with N1,N2 and N3 was(236.57±22.49),(274.24±25.21)and(361.45±33.35)μg/L respectively,which was significantly higher than that in N0[(185.21±17.36)μg/L,F=16.576,P<0.05].Compared with the positive expression rate of TNF-αin T1 stage gastric cancer(25.00%),the positive expression rate of TNF-αin gastric cancer patients with T2 stage(41.67%),T3 stage(52.38%)and T4 stage(77.78%)was significantly increased(χ^(2)=11.547,P<0.05).As compared with the positive expression rate of TNF-αin gastric cancer patients with T2 stage(41.67%),the positive expression rate of TNF-αin T3(52.38%)and T4(77.78%)stage was significantly increased(χ^(2)=9.252,P<0.05).There was no significant difference in the positive expression rate of TNF-αamong poorly differentiated group(50.00%),moderately differentiated group(50.00%)and well d
