手助腹腔镜与腹腔镜治疗胃间质瘤的临床疗效对比

摘　　要：目的比较手助腹腔镜与腹腔镜治疗胃间质瘤的临床疗效。方法回顾性分析2016年12月至2020年12月中国科学技术大学附属第一医院收治的53例确诊为胃间质瘤患者的临床资料,根据手术方法的不同,分为手助腹腔镜组(n=17)与腹腔镜组(n=36)。比较两组患者一般资料、手术时间、术中出血量、肿瘤最大切缘、术中有无破溃及术后通气时间、术后下床时间、术后进食时间、住院时间、住院费用、术后并发症及生存结果等的差异。结果手助腹腔镜组患者切除的肿瘤最大切缘为(2.28±0.93)cm,小于腹腔镜组,手术时间为(70.59±19.05)min,短于腹腔镜组,差异均有统计学意义(P<0.05),术中出血量为(58.33±20.38)mL,少于腹腔镜组,术后通气时间为(2.72±0.45)d、术后下床时间为(1.61±0.55)d、术后开始进食时间为(5.47±0.51)d,短于腹腔镜组,差异均有统计学意义(P<0.05)。结论相较腹腔镜手术,手助腔镜手术治疗胃间质瘤的手术时间短,患者术后恢复更快,且能有效保留更多的正常胃组织,同时不增加术后并发症。Objective To compare the clinical efficacy of hand-assisted laparoscopy and laparoscopy in the treatment of gastric stromal tumors,and to explore the clinical application value of hand-assisted laparoscopy in the treatment of gastric stromal tumors.Methods The clinical data of 53 patients with gastric stromal tumors in the First Affiliated Hospital of University of Science and Technology of China from December 2016 to December 2020 were retrospectively analyzed.Patients were divided into the hand-assisted laparoscopy group(n=17)and laparoscopy group(n=36)depended on the procedure.The operative time,intraoperative blood loss,maximum tumor margin intraoperative ulceration,time to first flatus,time to ambulation,time to first liquid intake,postoperative hospital stay,postoperative complications,hospital costs and survival outcome were compared between the two groups.Results The maximum margin of the hand-assisted laparoscopic group was(2.28±0.93)cm,less than that of the laparoscopic group,and the surgical time was(70.59±19.05)min,shorter than that of the laparoscopic group with statistically significant difference(P<0.05);the intraoperative bleeding was(58.33±20.38)mL,les s than that of laparoscopic group,postoperative ventilation time was(2.72±0.45)d,postoperative getting out-of-bed time was(1.61±0.55)d,and postoperative feeding time was(5.47±0.51)d,which were all less than those of the laparoscopic group,and the differences were statistically significant(P<0.05).Conclusions Compared with laparoscopic surgery,the operation time of hand-assisted surgery for gastric messtioma is short,the patient’s postoperative recovery is faster,and it can effectively retain more normal gastric tissue without increasing postoperative complications.
