房颤患者卒中后微栓子与血调节性T细胞、淋巴细胞表面凋亡因子相关性研究

摘　　要：目的探究房颤合并急性缺血性卒中患者的微栓子与调节性T细胞(Treg)和淋巴细胞表面凋亡因子(CD95)的关系。方法选取2015年10月至2021年1月威海市立医院全科医学科诊治的111例房颤合并急性缺血性卒中患者为试验组,同期选取73例不合并房颤的急性缺血性卒中患者为对照组,进行回顾性分析。监测所有研究对象的微栓子信号(MES),检测并比较两组患者外周血调节性T细胞(Treg)及淋巴细胞表面凋亡因子(CD95)水平。根据MES结果,将试验组分为MES阳性(28例)、阴性(83例)两个亚组,比较亚组间上述两指标水平的差异。结果试验组患者MES阳性率高于对照组(P<0.05),且CD95水平高于对照组、Treg水平低于对照组(P<0.05)。相较于MES阴性亚组,MES阳性亚组患者的Treg水平较低、CD95水平较高(均P<0.05)。结论房颤患者合并急性缺血性卒中更容易有微血栓形成,其免疫失调及炎症反应较不合并房颤的患者更为严重。Objective To investigate the changes of microemboli,Treg and CD95 in patients diagnosed with atrial fibrillation complicated with acute ischemic stroke.Methods 111 patients from October 2015 to January 2021 with atrial fibrillation complicated with acute ischemic stroke in department of General Medicine in Weihai Municipal Hospital of Shandong University were selected as the observation group,and 73 patients without atrial fibrillation as the control group.The changes of microembolic signal(MES),regulatory T cells(Treg)and apoptosis factor(CD95)in peripheral blood T lymphocytes were detected.Results The MES positive rate and the level of apoptosis factor(CD95)in the observation group were higher than that in the control group,and the level of regulatory T cells(Treg)was lower than that in the control group(P<0.05).Compared with MES negative patients,MES positive patients had lower Treg level and higher CD95 expressed by T lymphocytes(P<0.05).Conclusion Patients with atrial fibrillation at the acute phase of ischemic stroke were more likely to have microthrombosis,and their immune imbalance and inflammatory reaction may be more serious than those without atrial fibrillation.
