直肠癌组织中肿瘤浸润性树突状细胞数量和表型的改变及其临床意义

摘　　要：为分析直肠癌组织中肿瘤浸润性树突状细胞(TIDCs)数量和表型的变化,探讨其临床意义,以2018年1月至2019年12月我院收治的且行手术治疗的40例直肠癌患者为研究对象,术中分别取癌组织及其癌旁正常肠黏膜组织(距肿瘤缘>5cm),检测TIDCs数量及表型,进行对比分析,并分析直肠癌组织中TIDCs数量及表型与临床病理因素的关系。结果显示,直肠癌组织中TIDCs数量及MHC-Ⅱ阳性、CD54阳性树突状细胞(DC)比例均少于癌旁组织,P<0.05。TNM分期Ⅰ-Ⅱ期直肠癌患者的TIDCs数量及MHC-Ⅱ阳性、CD54阳性DC比例均多于Ⅲ期患者,P<0.05;肿瘤最大径<5cm者TIDCs数量及MHC-Ⅱ阳性、CD54阳性DC比例均多于≥5cm者,P<0.05。不同分化程度直肠癌患者各指标比较差异无统计学意义,P>0.05。结果表明,直肠癌组织中TIDCs呈低表达,不成熟细胞比例升高。该表现与肿瘤大小、分期有关,肿瘤越大、分期越高,TIDCs表达越低,不成熟细胞比例越高。This study was to investigate the amount and phenotype changes of tumor-infiltrating dendritic cells(TIDC)in rectal cancer tissue,and their clinical significance,enrolled 40 cases of rectal cancer treated in author’s hospital(2018-01-2019-12)surgically into this research:during operation,sample their cancer tissue and normal tissue close to cancer tissue(>5 cm far from neoplasm-edge),detect TIDCs amount and phenotype,then compare them,analyse the relationship of amount and phenotype changes with clinical pathdogical factors.As results,the TIDCs amount and the ratio of the DC of MHCⅡpositivity and CD54 positivity in rectal tissue were less than that in the tissue close cancer tissue(P<0.05);TIDCs amount and the ratio of DC of MHCⅡpositivity and CD54 positivity in the patients with TNMⅠ-Ⅱstgae of rectal cancer were more than that in the patients with TNMⅢof rectal cancer(P<0.05);TIDCs amount and the ratio of the DC of MHCⅡpositivity and CD54 positivity in the patients with max.diameter<5 cm were more than that in the patients with one≥5 cm(P<0.05);for patients at different diffentiation degree their all norms had no statistical difference each to other(P>0.05).Results show that in cancer tissue TIDCs show low expression,and the ratio of non-matured cell enhanced which are related to neoplasm site and staging,i.e the larger neoplasm is,and the higher staging is,while,the lower TIDCs expression is,and the higher non-matured cell ratio is.
