慢性萎缩性胃炎患者Hp感染与IL-8、PCT、TNF-α表达水平及严重程度的关系分析

摘　　要：目的探究慢性萎缩性胃炎(CAG)患者幽门螺杆菌(Hp)感染与白介素-8(IL-8)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)表达水平及病情严重程度的关系,为CAG早期诊治提供参考。方法收集2017年9月-2019年12月漯河市第六人民医院诊治的102例CAG患者(疾病组)和82例慢性浅表性胃炎患者(对照组)临床资料,胃镜下观察CAG患者胃萎缩严重程度,比较疾病组、对照组Hp阳性感染率、血清IL-8、PCT、TNF-α水平,同时分析CAG患者Hp感染与IL-8、PCT、TNF-α表达水平及病情严重程度的关系。结果疾病组Hp阳性感染率、血清IL-8、PCT、TNF-α水平较对照组明显升高,差异有统计学意义(P<0.05)。CAG患者Hp感染阳性者血清IL-8、PCT、TNF-α水平明显高于Hp感染阴性者,差异有统计学意义(P<0.05)。CAG患者病情严重者Hp阳性感染率、血清IL-8、PCT、TNF-α水平显著高于病情一般者,差异有统计学意义(P<0.05)。相关性分析显示,CAG患者Hp感染阳性率与血清IL-8、PCT、TNF-α水平及病情严重程度呈显著正相关(P<0.05)。结论血清IL-8、PCT、TNF-α水平或与Hp共同介导了CAG病理过程,对CAG早期诊治有一定的参考价值。Objective To explore the relationship between Helicobacter pylori(Hp) infection and expression levels of interleukin-8(IL-8),procalcitonin(PCT)and tumor necrosis factor-alpha(TNF-α)and the severity of chronic atrophic gastritis(CAG),and provide references for early diagnosis and treatment of CAG. Methods The clinical data of 102 patients with CAG(the disease group)and 82 patients with chronic superficial gastritis(the control group)who were diagnosed and treated in the hospital between July 2017 and December 2019 were collected. The severity of gastric atrophy in patients with CAG was observed under gastroscope. The positive rates of Hp infection and expression levels of IL-8,PCT and TNF-α were compared between the two groups. The relationship between Hp infection and expression levels of IL-8,PCT and TNF-α and the severity of CAG was analyzed. Results The positive rate of Hp infection and expression levels of IL-8,PCT and TNF-α in the disease group were significantly higher than those in the control group(P<0.05). In patients with CAG,the expression levels of IL-8,PCT and TNF-α were significantly higher in Hp positive patients than those in Hp negative patients(P<0.05). The positive rates of Hp infection and expression levels of IL-8,PCT and TNF-α were significantly higher in patients with severe CAG than those in normal condition(P<0.05). Correlation analysis indicated that the positive rate of Hp infection in patients with CAG was positively correlated with levels of serum IL-8,PCT and TNF-αas well as the severity of the disease(P<0.05). Conclusion Levels of serum IL-8,PCT and TNF-α together with Hp might mediate the pathological process of CAG,providing a valuable reference for early diagnosis and treatment of CAG.
