摘 要:目的探究磁共振扩散加权成像(DWI)在聚乙烯醇(PVA)微球肝动脉化疗栓塞疗效评估中应用价值。方法选择我院2015年12月至2020年6月期间收治80例行PVA微球肝动脉化疗栓塞治疗患者,在治疗前后均行DWI检查,评估其在患者治疗疗效中应用价值。结果80例患者中稳定期以及进展期分别为35、45例;病情稳定组患者治疗后表观扩散系数(ADC)值、肿瘤体积较治疗前显著改善(P<0.05),病情进展组治疗前后ADC值、肿瘤体积差异比较没有统计学意义(P>0.05),病情稳定组治疗后ADC值显著高于病情进展组,肿瘤体积显著低于病情进展组(P<0.05);ADC值与肿瘤体积显示为负相关关系(r=-0.419,P<0.05);ROC曲线显示ADC值评估患者疗效曲线下面积为0.850,敏感度与特异度分别为81.3%和86.9%。结论DWI测定ADC值与聚乙烯醇微球肝动脉化疗栓塞治疗患者肿瘤体积相关性较高,其用于评估患者疗效价值优异。Objective To explore the application value of diffusion-weighted imaging(DWI) in curative effect evaluation of hepatic artery chemoembolization with polyvinyl alcohol(PVA) micro-sphere.Methods Eighty patients undergoing hepatic artery chemoembolization with PVA micro-sphere in the hospital from December 2015 to June 2020 were enrolled as the research objects.All of them underwent DWI examination before and after treatment,and its application value in curative effect evaluation was evaluated.Results Of the 80 patients,35 were in the sta ble stage,and 45 were in the progressive stage.After treatment,the appa rent diffusion coefficient(ADC) and tumor volume of the stable group were improved significantly(P<0.05).There were no significant differences in ADC value and tumor volume in a progressive group before and after treatment(P>0.05).After treatment,the stable group had a significantly higher ADC value and significantly smaller tumor volume than the progressive group(P<0.05).The ADC value was negatively correlated with tumor volume(r=-0.419,P<0.05).ROC curves showed that the area under the curve(AUC),sensitivity,and specificity for curative effect evaluation were 0.850,81.3%,and 86.9%,respectively.In conclusion,the ADC value of DWI is highly related to tumor volume of patients undergoing hepatic artery chemoembolization with PVA microsphere.Therefore,it is of high value in curative effect evaluation.
