摘　　要：目的探讨乳腺导管原位癌(Ductal carcinoma in situ,DCIS)微钙化X线生物学特征与病理分级的相关性。方法选择2017年1月至2019年12月于在本院经钼靶定位穿刺微钙化病灶并通过手术、病理证实的DCIS患者101例,按照钙化形态分为细小多形性组(M1组)与非细小多形性组(非M1组),按照钙化分布分为群集分布组(D1组)与非群集分布组(非D1组)。分析患者钙化形态、分布与ER、HER2、ER/HER2亚型、Ki-67表达及病理分级的相关性。结果M1组与非M1组患者的ER、HER2、ER/HER2亚型、Ki-67表达及病理分级之间差异无统计学意义(P>0.05);D1组与非D1组患者ER、HER2、ER/HER2亚型、Ki-67表达及病理分级之间差异有统计学意义(P<0.05);X线生物学特征与病理分级的之间存在负向弱相关性(r=-0.391)。结论DCIS患者钙化分布与ER、HER2、ER/HER2亚型、Ki-67表达及病理分级具相关性,能为预后评估提供参考。Objective To investigate the correlation between the X-ray biological characteristics of breast ductal carcinoma in situ(DCIS)micro-calcification and pathological grade.Methods From January 2017 to December 2019,101 patients with DCIS who were punctured with mammography target and confirmed by surgery and pathology were selected.The patients were divided into small pleomorphic group(M1 group)and non-small pleomorphic group(non-M1 group)according to calcification form and divided into clustered distribution group(D1 group)and non-clustered distribution group(non-D1 group)according to the calcification distribution.The correlation between the calcification form and distribution of patients with ER,HER2,ER/HER2 subtype,Ki-67 expression and pathological grade were analyzed.Results The ER,HER2,ER/HER2 subtype,Ki-67 expression and pathological grade of M1 group and non-M1 group had no significant difference;The ER,HER2,ER/HER2 subtype,Ki-67 expression and pathological grade of D1 group and non-D1 group had significant differences.There is a negative weak correlation between the X-ray biological characteristics and pathological grade(r=-0.391).Conclusion The distribution of calcification in DCIS patients is correlated with ER,HER2,ER/HER2 subtypes,Ki-67 expression and pathological grade,which can provide references for prognostic evaluation.
