在宫颈癌筛查中开展阴道镜下宫颈活组织检测与病理诊断结果的比较

摘　　要：目的:比较在宫颈癌筛查中阴道镜下宫颈活组织检测与病理诊断的结果。方法:选取2018年6月-2020年5月进行宫颈癌筛查的患者600例,所有患者均进行阴道镜下宫颈活组织检测及病理检查。以病理检查诊断结果为临床标准,比较阴道镜下宫颈活组织检测的诊断准确率。结果:与病理检查诊断结果相比,阴道镜下宫颈活组织检测的诊断符合率为81.50%,两种检查结果比较,差异有统计学意义(P<0.05)。其中慢性宫颈炎经过病理检查升级为CIN为2.88%,扁平湿疣、湿疣样病变经过病理检查升级为CIN为2.23%,CIN1级经过病理检查升级为CIN2级与3级为10.74%,升级为微小浸润癌为1.65%;CIN2级与3级经过病理检查升级为微小浸润癌为14.29%。结论:在宫颈癌的筛查中采用阴道镜下宫颈活组织检测会存在一定的微小浸润癌漏诊风险,且具有一定的误诊率,对阴道镜下宫颈活组织检测为较高CIN级别的患者,需结合病理检查提升诊断准确率。Objective:To compare the results of cervical biopsy under colposcopy and pathological diagnosis in cervical cancer screening.Methods:From June 2018 to May 2020,600 patients who underwent cervical cancer screening were selected,all of them underwent cervical biopsy under colposcopy and pathological examination.The diagnosis result of pathological examination was used as the clinical standard to compare the diagnostic accuracy of cervical biopsy under colposcopy.Results:Compared with the diagnosis result of pathological examination,the diagnosis coincidence rate of cervical biopsy under colposcopy was 81.50%,and the difference between the two examination results was statistically significant(P<0.05).Among them,2.88%of chronic cervicitis was upgraded to CIN through pathological examination,2.23%of flat condyloma and condyloma-like lesions were upgraded to CIN through pathological examination,CIN level 1 was upgraded to CIN level 2 and level 3 were 10.74%.1.65%were micro-invasive carcinomas.14.29%of CIN grades 2 and 3 were upgraded to micro-invasive carcinomas after pathological examination.Conclusion:The use of cervical biopsy under colposcopy in the screening of cervical cancer will have a certain risk of missed diagnosis of microscopic invasive cancer,and has a certain misdiagnosis rate.For patients with a higher CIN level under colposcopy cervical biopsy,it is necessary to combine pathological examinations to improve the accuracy of diagnosis.
