摘　　要：目的:分析卵巢恶性畸胎瘤的声像图特征,探讨超声诊断价值,并分析误诊病例。方法:回顾性分析2016年5月—2020年12月浙江大学医学院附属妇产科医院22例经手术病理证实为卵巢恶性畸胎瘤的超声资料,总结其声像图特征并分析误诊病例。结果:22例中19例(19/22,86.4%)为未成熟畸胎瘤,3例(3/22,13.6%)为成熟畸胎瘤恶变,均为囊实性肿块:囊性为主型7例(7/22,31.8%)、囊实混合型11例(11/22,50.0%)、实性为主型4例(4/22,18.2%);彩色多普勒血流分级:0级2例(2/22,9.1%)、Ⅰ级12例(12/22,54.5%)、Ⅱ级8例(8/22,36.4%)。术前超声诊断为恶性畸胎瘤及恶性卵巢肿瘤15例(15/22,68.2%),其他诊断7例(7/22,31.8%):包括成熟畸胎瘤4例、囊腺瘤2例、黄体破裂1例。结论:卵巢恶性畸胎瘤超声表现复杂多变,术前诊断符合率相对较低,但仍具有一定的特征性。Objective To analyze the ultrasonographic characteristics of ovarian malignant teratoma,explore the value of ultrasound diagnosis and analyze the misdiagnosed cases.Methods Retrospective analysis of the ultrasound data of 22 cases of ovarian malignant teratoma confirmed by surgery and pathology in the Women’s Hospital,Zhejiang University School of Medicine from May 2016 to December 2020.The ultrasonographic features were summarized and the misdiagnosed cases were analyzed.Results Of the 22 cases,19(19/22,86.4%)were immature teratoma and 3(3/22,13.6%)were malignant transformation of mature teratoma,all of which showed cystic and solid masses:cystic type in 7 cases(7/22,31.8%),mixed cystic and solid type in 11 cases(11/22,50.0%)and solid type in 4 cases(4/22,18.2%).Color Doppler flow classification showed 2 cases of grade 0(2/22,9.1%),12 cases of grade Ⅰ(12/22,54.5%),and 8 cases of grade Ⅱ(8/22,36.4%).Preoperative ultrasound diagnosis of 15 cases of malignant teratoma or malignant ovarian tumor(15/22,68.2%),other diagnoses of 7 cases(7/22,31.8%):including 4 cases of mature teratoma and 2 cases of cystadenoma and 1 case of corpus luteum rupture.Conclusion The ultrasound manifestations of ovarian malignant teratoma are complex and changeable,and the preoperative diagnosis coincidence rate is relatively low,but it still has certain characteristics.
