多发性骨髓瘤患者骨髓细胞周期素D1、D3、p21的表达及与预后的相关性研究

摘　　要：目的:检测多发性骨髓瘤患者骨髓中细胞周期素D1(Cyclin D1)、细胞周期素D3(Cyclin D3)、p21的表达情况,并探究其表达水平与多发性骨髓瘤患者预后的相关性。方法:选取2015-07—2016-12在本院血液科确诊并住院治疗的多发性骨髓瘤患者80例为多发性骨髓瘤组,选取同期本院诊断为营养性贫血患者20例为对照组。采用实时荧光定量PCR法(qRT-PCR)检测所有研究对象骨髓中Cyclin D1、D3、p21 mRNA表达水平;通过Kaplan-Meier法绘制生存曲线,评估骨髓中Cyclin D1、D3、p21表达水平对多发性骨髓瘤患者预后的影响;采用Cox回归分析多发性骨髓瘤患者预后的影响因素。结果:多发性骨髓瘤组Cyclin D1、D3 mRNA表达水平均显著高于对照组(P<0.05),p21 mRNA表达水平显著低于对照组(P<0.05)。Cyclin D1、D3高表达组术后3年总生存率低于Cyclin D1、D3低表达组(P<0.05),p21高表达组术后3年总生存率高于p21低表达组(P<0.05)。Cox回归分析结果显示,Cyclin D1、D3高表达、p21低表达是影响多发性骨髓瘤患者预后的独立危险因素(HR=2.884,95%CI为1.714-4.853;HR=2.575,95%CI为1.266-5.239;HR=2.979,95%CI为1.671-5.312;P均<0.05)。结论:Cyclin D1、D3、p21或可作为多发性骨髓瘤的病情评估及不良预后预测的的潜在生物学指标。Objective:To detect the expressions of Cyclin D1,Cyclin D3 and p21 in bone marrow of patients with multiple myeloma,and to explore the correlation between the expression levels and prognosis of patients with multiple myeloma.Method:From July 2015 to December 2016,80 patients with multiple myeloma diagnosed and hospitalized in our hospital were selected as multiple myeloma group,and 20 patients with nutritional anemia in our hospital were selected as control group.Real-time fluorescent quantitative PCR(qRT-PCR)was used to detect the mRNA expression levels of Cyclin D1,D3 and p21 in bone marrow of all subjects.Kaplan-Meier method was used to draw survival curve to evaluate the effect of Cyclin D1,D3 and p21 expression levels on the prognosis of patients with multiple myeloma.In addition,Cox regression analysis was used to analyze the prognostic factors of multiple myeloma patients.Results:The expression level of Cyclin D1,D3 mRNA in multiple myeloma group was significantly higher than that in control group(P<0.05),and expression level of p21 mRNA was significantly lower than that in control group(P<0.05).The 3-year overall survival rate of Cyclin D1,D3 high expression group was lower than that of Cyclin D1,D3 low expression group(P<0.05),and that of p21 high expression group was higher than that of p21 low expression group(P<0.05).Cox regression analysis showed that high expression of Cyclin D1,D3 and low expression of p21 were independent risk factors for prognosis of patients with multiple myeloma(HR=2.884,95%CI:1.714-4.853;HR=2.575,95%CI:1.266-5.239;HR=2.979,95%CI:1.671-5.312;P<0.05).Conclusion:Cyclin D1,D3 and p21 may be used as potential biological indicators for disease evaluation and poor prognosis prediction of multiple myeloma.
