摘　　要：目的探讨白内障手术前后角膜地形图及泪液分泌变化。方法82例行白内障超声乳化摘除和人工晶体植入术的患者作为研究对象,对手术前后的角膜地形图及泪液分泌变化进行分析。结果术后1 d,患者的角膜表面非对称性指数(SAI)、角膜表面规则性指数(SRI)均高于术前,差异有统计学意义(P<0.05);术后1周、1个月,患者的SAI、SRI与术前比较,差异均无统计学意义(P>0.05)。术后1 d、1周、1个月,患者的泪膜破裂时间(BUT)均短于术前,基础泪液分泌实验(SⅠT)均低于术前,差异有统计学意义(P<0.05)。术后1 d、1周、1个月,患者的角膜内皮细胞计数(CEC)分别为(2216.71±172.58)、(2243.54±178.39)、(2269.76±183.67)个/mm^(2),均低于术前的(2331.35±201.77)个/mm^(2);裸眼视力(UCVA)分别为(0.36±0.08)、(0.42±0.06)、(0.63±0.08),均高于术前的(0.17±0.09),差异有统计学意义(P<0.05)。结论白内障手术对患者术后早期内皮细胞、角膜地形图和泪液分泌均会产生一定的影响,但能有效改善患者视力,术后恢复较快。Objective To discuss the changes of corneal topography and tear secretion before and after surgery.Methods 82 cataract patients who underwent phacoemulsification and intraocular lens implantation as study subjects,the changes of corneal topography and teat secretion before and after surgery were analyzed.Results 1 d after surgery,the surface asymmetry index(SAI)and surface regularity index(SRI)of patients were higher than those before surgery,and the difference was statistically significant(P<0.05).1 week and 1 month after surgery,the SAI and SRI of patients had no statistically significant difference compared with those before surgery(P>0.05).1 d,1 week and 1 month after surgery,the break-up time(BUT)of patients was shorter than that before surgery,and Schirmer Ⅰ test(SⅠT)was lower than that before surgery.All the difference was statistically significant(P<0.05).1 d,1 week and 1 month after surgery,the corneal endothelium count(CEC)of patients were(2216.71±172.58),(2243.54±178.39)and(2269.76±183.67)cells/mm^(2),which were lower than(2331.35±201.77)cells/mm^(2) before surgery,and uncorrected visual acuity(UCVA)were(0.36±0.08),(0.42±0.06)and(0.63±0.08),which was higher than(0.17±0.09)before surgery.All the difference was statistically significant(P<0.05).Conclusion Cataract surgery can affect endothelial cells,corneal topography and tear secretion in the early postoperative stage,but it can effectively improve the visual acuity of patients,and the postoperative recovery is faster.
