术前区域性动脉化疗栓塞联合全身静脉化疗对局部进展期胃癌的效果分析

摘　　要：目的探讨术前区域性动脉化疗栓塞联合全身静脉化疗对局部进展期胃癌的疗效。方法收集2013年8月至2017年8月郑州大学第一附属医院收治的84例经新辅助治疗后接受胃癌根治术的局部进展期胃癌患者的临床资料。术前接受区域性动脉化疗栓塞联合全身静脉化疗患者41例(联合治疗组),术前接受单纯全身静脉化疗患者43例(静脉化疗组)。比较两组患者新辅助治疗效果、不良反应发生情况、手术情况、术后并发症发生情况及预后。结果联合治疗组临床反应率高于静脉化疗组,差异有统计学意义(P<0.05);联合治疗组恶心呕吐、神经毒性不良反应发生率高于静脉化疗组,差异有统计学意义(P<0.05);两组患者术后并发症发生率比较,差异无统计学意义(P>0.05);联合治疗组3 a复发转移率低于静脉化疗组,差异有统计学意义(P<0.05);联合治疗组3 a生存率高于静脉化疗组,差异有统计学意义(P<0.05)。结论术前区域性动脉化疗栓塞联合全身静脉化疗治疗局部进展期胃癌效果确切,可以有效降低患者胃癌分期,降低术后复发、转移率,提高患者3 a生存率。Objective To investigate the effect of preoperative regional arterial chemoembolization combined with systemic intravenous chemotherapy on locally advanced gastric cancer.Methods The clinical data of 84 patients with locally advanced gastric cancer who underwent radical gastrectomy after neoadjuvant therapy in the First Affiliated Hospital of Zhengzhou University from August 2013 to August 2017 were collected.There were 41 patients who received regional arterial chemoembolization combined with systemic intravenous chemotherapy before operation(combined treatment group),and 43 patients who received only systemic intravenous chemotherapy before operation(intravenous chemotherapy group).The effect of neoadjuvant therapy,adverse reactions,operation,postoperative complications and prognosis were compared between the two groups.Results The clinical response rate of the combined treatment group was higher than that of the intravenous chemotherapy group,and the difference was statistically significant(P<0.05).The incidence of nausea,vomiting and neurotoxicity in the combined treatment group was higher than that in the intravenous chemotherapy group(P<0.05).There was no statistically difference in the incidence of postoperative complications between the two groups(P>0.05).The 3 a recurrence and metastasis rate of the combined treatment group was lower than that of the intravenous chemotherapy group,and the difference was statistically significant(P<0.05).The 3 a survival rate of the combined treatment group was higher than that of the intravenous chemotherapy group,and the difference was statistically significant(P<0.05).Conclusion Preoperative regional arterial chemoembolization combined with systemic intravenous chemotherapy is effective in the treatment of locally advanced gastric cancer,which can effectively reduce the staging of gastric cancer,reduce the recurrence and metastasis rate,and improve the 3-year survival rate of patients.
