摘 要:目的探讨T_(2) mapping在甲状腺相关性眼病(TAO)早期视神经改变评估中的价值。方法回顾性分析行T_(2) mapping检查的30例TAO患者(非甲状腺相关视神经病变患者)(病例组)及15例健康志愿者(对照组)的临床和影像学资料。测量并比较2组间视神经T_(2)弛豫时间(T_(2)RT)值、眼球突出度,眼外肌最大径以及临床参数的差异。对病例组视神经TRT值与各临床及影像参数行相关分析。采用受试者工作特征(ROC)曲线法评价T_(2)RT值对TAO与对照组视神经的鉴别效能。结果病例组视神经T_(2)RT值大于对照组(P<0.001)。病例组视神经T_(2)RT值与临床活动度评分(CAS)(r=0.640,P<0.001),眼球突出度(r=0.433,P=0.001)及眼外肌最大径(r=0.491,P<O.001)呈正相关。以T_(2)RT值>82.9 ms判断TAO视神经异常,可获得最优的诊断效能(敏感度0.717;特异度0.667;曲线下面积0.727)。结论TRT值可反映视神经改变,T_(2) mapping技术可用于评估TAO患者早期祧神经异常,对于临床洽疗具有指导意义。Objective explore the value of T_(2) mapping in assessing optic nerve changes in patients with thyroid-associated ophthalmopathy(TAO).Methods Thirty TAO patients and 15 healthy controls(HC)who underwent T_(2) mapping scan were enrolled in our study.The differences of T_(2) relaxation time(T_(2) RT)of optic nerve,exophthalmos,maximum thickness of extraocular muscles and clinical parameters were measured and compared between TAO and HC.Correlations between T_(2) RT value and clinical,radiological parameters were analyzed.Receiver operating characteristic(ROC)curve analysis was used to determine the differentiating ability of T_(2)RT value.Results The T_(2)RT value of optic nerve in the TAO was significantly higher than that of the HC(P<0.001).T_(2)RT value of optic nerve was positively correlated with clinical activity score(CAS)(r=0.640,PV0.001),exophthalmos(r=0.433,P=0.001)and maximum thickness of extraocular muscles(r=0.491,P<0.001).Set ting a T_(2) RT value of 82.9 ms as the cut-off value,optimal distinguishing performance between TAO and HC was achieved with the sensitivity of 0.717,the specificity of 0.667,and the area under curve of 0.727.Conclusion T_(2) RT value could reflect the changes of optic nerves.and T_(2) mapping can be used to assess the early optic nerve abnormality in TAO patients,which is significant to guide clinical treatment.
