卵巢卵泡膜-纤维瘤与颗粒细胞瘤的CT鉴别诊断

摘　　要：目的分析卵巢卵泡膜纤维瘤(OT-F)与卵巢颗粒细胞瘤(OGCT)的CT影像特征,提高两者的CT诊断准确率。方法回顾性分析经手术及病理证实的34例OT-F和12例OGCT患者的临床及CT资料,分析肿瘤数目、位置、大小、形态、成分、强化程度及伴随征象等在两者中的分布差异。结果34例OT-F共35个肿瘤,12例OGCT共13个肿瘤;0T-F和OGCT中实性囊性爽实性分别为30/35、0/35、5/35,1/13.5/13.7/13(χ^(2)=28.33,P<0.001);OT-F和OGCT的平扫CT值分别为(39.83±9.16)HU、(34.08±5.12)HU(t=2135,P=0.038),两者的CT强化值为(9.26±8.77)HU、(28.23±8.18)HU(t=6.78,P<0.001);OT-F和OGCT合并瘤内出血分别为0/35.7/13(χ^(2)=22.06,P<0.001),两者伴子宫内膜异常的病例分别为7/34.7/12(χ^(2)=5.256,P=0.022)。OT-F和OGCT在肿瘤的大小、位置、形态及合并肿瘤、积液、钙化等特征中的差异无统计学意义(P>0.05)。结论OT-F和OGCT在CT征象.上有一定的差异,OGCT以囊性或囊实性多见,平扫CT值低于OT-F,但强化程度明显高于OT-F,OGCT易伴发瘤内出血及子宫内膜异常。Objective To analyze the CT features of ovarian thecoma-fibroma(OT-F)and ovarian granulose cell tumor(OGCT),and to improve the diagnostic accuracy.Methods The clinical and CT data of 34 patients with OT-F and 12 patients with OGCT confirmed by surgery and pathology were analyzed retrospectively.The imaging features of the two entities,including amount,location,size,morphology,components,enhancement type and accompanying signs were recorded and compared.Results Totally,35 lesions of OT-F in 34 cases and 13 lesions of OGCT in 12 cases were enrolled in this study.The statistically differentiated imaging features between OT-F and OGCT were as follows respectively:the amounts of solid,cystic and cystic solids and were 30/35,0/35,5/35 for OT-F and 1/13,5/13,7/13 for OGCT(χ^(2)=28.33,P<0.001);the CT value of plain scan were(39.83±9.16)HU for OT-F and(34.08±5.12)HU for OGCT(t=2.135,P=0.038);the CT enhancement value were(9.26±8.77)HU for OT-F and(28.23±8.18)HU for OGCT(t=6.78,P<0.001);OT-F and OGCT combined with intratumoral hemorrhage were 0/35 and 7/13(χ^(2)=22.06,P<0.001),the cases of endometrial abnormalities were 7/34 and 7/12(χ^(2)=5.256,P=0.022).There was no significant difference in the size,location,morphology,and combined tumor,effusion,and calcification of OT-F and OGCT(P>0.05).Conclusion OT-F and OGCT have certain differences in CT signs.OGCT is more common in cystic or cystic,and the CT value of plain scan is lower than OT-F,but the degree of enhancement is significantly higher than that of OT-F.In addition,OGCT is more likely to be accompanied with intratumoral hemorrhage and abnormal endometrium.
