双歧杆菌三联活菌联合铋剂四联疗法治疗Hp阳性合并消化道溃疡的疗效

摘　　要：目的研究双歧杆菌三联活菌联合铋剂四联疗法治疗幽门螺杆菌(Hp)阳性合并消化道溃疡的疗效。方法76例Hp阳性合并消化道溃疡的患者,以双色球法随机分为观察组与对照组,各38例。对照组行常规铋剂四联疗法治疗,观察组行双歧杆菌三联活菌联合铋剂四联疗法治疗。对比两组血锌浓度及胃液pH值,炎症反应改善情况,Hp转阴以及不良反应发生情况。结果观察组患者血锌浓度(90.93±11.54)μmol/L、胃液pH值(6.32±1.02)均高于对照组的(81.75±12.60)μmol/L、(4.70±0.89),差异有统计学意义(P<0.05)。观察组肿瘤坏死因子-α、白介素-8均低于对照组,差异有统计学意义(P<0.05)。观察组Hp转阴率84.21%高于对照组的44.74%,差异有统计学意义(P<0.05)。观察组不良反应总发生率5.26%低于对照组的23.68%,差异有统计学意义(P<0.05)。结论联合双歧杆菌三联活菌以及铋剂四联疗法治疗Hp阳性合并消化道溃疡患者,可有效改善其血锌浓度以及胃部酸碱平衡状态,减轻炎症反应,促进Hp转阴,安全性高,临床治疗效果良好,值得推广。Objective To study the efficacy of Bifidobacterium triple viable combined with bismuthbased quadruple therapy in the treatment of Helicobacter pylori(Hp)positive combined with peptic ulcer.Methods A total of 76 cases of Hp positive combined with peptic ulcer were divided into observation group and control group according to double color ball method,with 38 cases in each group.The control group was treated with bismuth-based quadruple therapy,and the observation group was treated with Bifidobacterium triple viable and bismuth-based quadruple therapy.The blood zinc concentration,gastric juice pH value,improvement of inflammatory response,Hp negative-conversion and occurrence of adverse reactions were compared between the two groups.Results The blood zinc concentration(90.93±11.54)μmol/L and gastric juice pH value(6.32±1.02)of the observation group were all higher than(81.75±12.60)μmol/L and(4.70±0.89)of the control group,and the difference was statistically significant(P<0.05).The tumor necrosis factor-αand interleukin-8 of the observation group were all lower than those of the control group,and the difference was statistically significant(P<0.05).The Hp negative-conversion rate 84.21%of the observation group was higher than 44.74%of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.26%,which was lower than 23.68%in the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of Bifidobacterium triple viable and bismuth-based quadruple therapy has good clinical treatment effect on Hp positive patients with peptic ulcer,and it can effectively improve the blood zinc concentration and gastric acid-base balance,reduce the inflammatory reaction,promote Hp negativeconversion.It has high safety,and is worthy of promotion.
