摘 要:目的探讨睡眠剥夺脑电图检测对小儿癫痫的诊断价值及其影响因素。方法选取2019年2月至2020年11月我院拟诊为癫痫的儿童患者124例,所有患儿均先使用脑功能监护仪检测脑功能。常规脑电图测试后2周进行睡眠剥夺脑电图检测。没有癫痫样放电意味着阴性,反之意味着阳性。观察睡眠剥夺脑电图检测的阳性率。采用多元线性回归分析对睡眠剥夺检测小儿癫痫阳性率的影响因素进行分析。结果睡眠剥夺脑电图检测癫痫样放电的患儿例数为100例,阳性率为80.7%,睡眠剥夺脑电图检测没有癫痫样放电的患儿例数为24例,阴性率为19.4%;睡眠剥夺脑电图检测的阴性患儿与阳性患儿的发作持续时长、发作次数临床资料比较差异有统计学意义(P<0.05);以睡眠剥夺脑电图检测小儿癫痫是否阳性作为因变量,将发作持续时长、发作次数作为自变量,通过多元线性回归分析,结果显示,发作持续时长、发作次数均是睡眠剥夺脑电图检测小儿癫痫阳性率的独立影响因素。结论睡眠剥夺脑电图检测对小儿癫痫的诊断价值较高,可提高诊断的阳性率,发作持续时长、发作频率是影响剥夺睡眠脑电图检测小儿癫痫阳性率的独立影响因素,故在诊断时应全面考虑上述两个影响因素,以提升诊断阳性率。Objective To investigate the diagnostic value of sleep-deprivation electroencephalography(EEG)in children with epilepsy and its influencing factors.Methods A total of 124 children,who were diagnosed with suspected epilepsy in our hospital between February 2019 and November 2020,were included in the study.All children were first tested by brain function monitor.Sleep-deprivation EEG was measured at 2 weeks after routine EEG test.The absence of epileptic discharges meant negative,otherwise it meant positive.The positive rate of sleep deprivation EEG was examined.Multiple linear regression analysis was used to analyze the factors influencing the positive rate of epilepsydetected by sleep-deprivation electroencephalographyin children.Results The number of children with epileptiform discharge detected by sleep deprivation EEG was 100,with a positive rate of 80.6%.The number of children without epileptiform discharge detected by sleep deprivation EEG was 24,with a negative rate of 19.4%.There were statistically significant differences in the duration and frequency of seizures between children with negative and positive results detected by sleep-deprivation EEG(both P<0.05).The sleep-deprivation EEG was used as a dependent variable to detect whether children with epilepsy were positive,and the duration and frequency of seizures were used as independent variables.Through multiple linear regression analysis,the results showed that the duration of seizures and frequency of seizures were independent influencing factors for the positive rate ofepilepsydetected by sleep-deprivation EEG in children.Conclusion Sleep-deprivation EEG is of high diagnostic value for children with epilepsy,and it may improve the positive rate of diagnosis.The duration and frequency of seizures are independent influencing factors for the positive rate of epilepsy detected by sleep-deprivation EEG in children.Therefore,the two factors should be fully considered in the diagnosis to increase the positive rate of diagnosis.
