摘 要:目的分析独立生长因子1B(GFI1B)在急性髓系白血病(AML)中的表达及对临床疗效的评估价值。方法回顾性分析2015年5月—2018年3月本院收治的106例AML为观察组,另取同期100例健康体检者为对照组。比较2组GFI1B表达情况,分析影响AML患者预后生存的危险因素。结果观察组GFI1B表达水平高于对照组(P<0.01)。与治疗前比较,治疗后完全缓解AML与部分缓解AML患者GFI1B表达水平显著降低,且完全缓解AML患者GFI1B表达水平低于部分缓解和难治AML复发患者,部分缓解AML患者的GFI1B表达水平亦低于难治复发AML患者(P<0.05,P<0.01)。难治复发AML患者治疗前后GFI1B表达水平比较无统计学意义(P>0.05)。白细胞水平、髓外浸润、危险度分型、达完全缓解疗程数、查尔森合并症指数(CCI)评分及GFI1B表达情况与AML患者预后生存呈正相关(P<0.05)。有髓外浸润、CCI评分≥2分及GFI1B表达≥0.5×10^(6)拷贝/μl cDNA是影响AML患者预后生存的独立危险因素(P<0.05)。结论GFI1B在AML患者中呈高表达,GFI1B异常升高者预后死亡风险更高,临床可加强对AML患者GFI1B的动态监测,以提高生存率。Objective To analyze the expression of growth factor-independent 1B(GFI1B)in acute myelogenous leukemia(AML)and its value in evaluation of clinical efficacy.Methods A total of 106 AML patients admitted between May 2015 and March 2018 were enrolled as observation group,while other 100 healthy people who had undergone physical examination at the same period were selected as control group.GFI1B expression was compared in both groups,and related risk factors that affected the prognosis and survival of AML patients were analyzed.Results The GFI1B expression in observation group was significantly higher than that in control group(P<0.01).After treatment,GFI1B expressions were significantly lower than those before treatment in AML patients with complete remission and partial remission,and GFI1B expression in AML patients with complete remission was significantly lower than those in patients with partial remission and refractory relapses,while GFI1B expression in patients with partial remission was also significantly lower than that in patients with refractory relapses(P<0.05,P<0.01).No significant difference was found in the GFI1B expression in patients with refractory relapse before and after treatment(P>0.05).Level of white blood cell,extramedullary infiltration,risk classification,the number of treatment courses to complete remission,Charlson comorbidy index(CCI)score and GFI1B expression were positively correlated with prognoses and survival of AML patients(P<0.05).Extramedullary infiltration,CCI score equal or more than 2 points and GFI1B expression equal or more than 0.5×10^(6) copies/μl cDNA were independent risk factors affecting prognoses and survival of AML patients(P<0.05).Conclusion GFI1B is highly expressed in AML patients,and patients with abnormally increased GFI1B expressions have higher prognostic death risk.Dynamic monitoring of GFI1B should be strengthened in order to improve survival rate in AML patients in clinical practice.
