糖类抗原125、癌胚抗原和神经特异性烯醇酶联合检测在食管鳞癌诊断中的应用价值

摘　　要：目的探讨血清糖类抗原125(CA125)、癌胚抗原(CEA)和神经特异性烯醇酶(NSE)联合检测用于食管鳞癌(ESCC)诊断的价值。方法选取郑州大学第一附属医院2018年11月至2020年10月收治的84例ESCC患者作为ESCC组,另选择84例健康体检者作为对照组。采用电化学发光法检测血清CA125、CEA和NSE水平。采用受试者工作特征曲线(ROC)评价单独或联合检测CA125、CEA和NSE诊断ESCC的敏感性、特异性和曲线下面积(AUC)。结果ESCC组患者血清CA125、CEA和NSE水平均显著高于对照组健康体检者,差异均有统计学意义(t=9.156,P<0.001;t=6.947,P<0.001;t=8.987,P<0.001)。采用CA125、CEA、NSE或CA125+CEA+NSE联合检测诊断ESCC的敏感性分别为0.786、0.774、0.845、0.881,特异性分别为0.750、0.810、0.726、0.821,约登指数分别为0.584、0.536、0.571、0.702,AUC分别为0.834、0.824、0.847和0.904。结论CA125、CEA和NSE在ESCC患者血清中的表达水平均上调,三者联合检测在ESCC中具有良好的诊断效能。Objective To investigate the value of serum carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA)and neuron specific enolase(NSE)in the diagnosis of esophageal squamous cell carcinoma(ESCC).Methods A total of 84 ESCC patients admitted to the First Affiliated Hospital of Zhengzhou University from November 2018 to October 2020 were selected as the ESCC group,and 84 healthy subjects were selected as the control group.The electrochemiluminescence method was used to determine the serum levels of CA125,CEA and NSE.The receiver operating characteristic(ROC)curve was used to evaluate the sensitivity,specificity and area under the curve(AUC)of CA125,CEA and NSE alone or in combination in the diagnosis of ESCC.Results The serum levels of CA125,CEA and NSE of patients in the ESCC group were significantly higher than those in the control group,the difference was statistically significant(t=9.156,P<0.001;t=6.947,P<0.001;t=8.987,P<0.001).Using CA125,CEA,NSE or CA125+CEA+NSE to diagnose esophageal cancer,the sensitivity was respectively 0.786,0.774,0.845,0.881,the specificity was respectively 0.750,0.810,0.726,0.821,the Youden index was respectively 0.584,0.536,0.571,0.702,and the AUC was respectively 0.834,0.824,0.847,0.904.Conclusion The serum expression levels of CA125,CEA and NSE of ESCC patients are significantly up-regulated,and the combined diagnosis of the three indicators has good diagnosis efficacy in the ESCC.
