摘　　要：目的探讨结缔组织移植物对上颌骨前牙种植术后周围组织影响及美学的影响。方法回顾性分析我院2017年1月~2018年1月收治的43例单颗上前牙缺失行前牙种植术患者为研究对象,根据是否行结缔组织移植分为对照组(二期手术未行结缔组织移植)20例、观察组(二期手术行结缔组织移植)23例。分析观察组植入物存活情况,比较两组患者软组织厚度、角化黏膜高度、临床冠长度、骨组织稳定性、唇侧丰满度、红色美学评分(PES)。结果随访1年,23例二期手术行结缔组织移植患者中22例组织瓣移植存活,存活率95.65%,23例患者取瓣区均未发生感染,黏膜愈合良好;术后6个月、术后1年两组角化黏膜高度比较无明显差异(P>0.05),观察组软组织厚度明显高于对照组(P<0.05),观察组临床冠长度明显低于对照组(P<0.05);术后6个月、术后1年观察组唇侧丰满度明显高于对照组(P<0.05);术后6个月、术后1年观察组PES评分明显高于对照组(P<0.05);术后6个月、术后1年观察组ISQ明显高于对照组(P<0.05)。结论对上颌骨前牙种植术后采用结缔组织移植可显著改善周围组织稳定性,提高唇侧丰满度,增加骨组织稳定性,获得较好美学效果。Objective To investigate the effect of connective tissue graft on the surrounding tissues after maxillary anterior dental implantation and its aesthetics effect.Methods A total of 43 patients with single upper anterior tooth loss in our hospital from January 2017 to January 2018 were enrolled,and divided into two groups according to the absence or presence of connective tissue graft,control group(n=20)and observation group(n=23).The survival of the implants in the observation group was analyzed,and the soft tissue thickness,keratinized mucosa,clinical crown length,implant stability quotient(ISQ),labial convexity score(LCS),and pink esthetic score(PES)were compared between groups.Results During the 1-year follow-up,connective tissue graft of 22 cases in observation group survived,with a survival rate of 95.65%.None of the 23 patients had infection in the donor site,and the mucosa healed well.At postoperative 6 mon and 1 year,the keratinized mucosa showed no significant difference in both groups,while observation group had thicker soft tissues and longer clinical crown length than those of control group(P<0.05).Compared with control group,observation group had had higher labial convexity score,PES score and ISQ score at postoperative 6 mon and 1 year(all P<0.05).Conclusion Application of connective tissue graft can effectively improve the stability of surrounding tissues and the labial convexity score,with better skeletal stability and good aesthetics effect.
