下颌近中阻生第三磨牙和第二磨牙远中骨丧失情况的相关关系研究

摘　　要：目的探讨近中阻生下颌第三磨牙(MTM)和第二磨牙远中牙槽骨丧失(MSMDBL)的相关关系。方法选取274例近中阻生MTM的CBCT资料,测量MTM与MSM牙长轴夹角、MTM近中釉牙骨质界至MSM远中釉牙骨质界距离(MTMMCEJ-MSMDCEJ)和MSMDBL,并统计年龄、性别,分析各因素对MSMDBL的影响及相关关系。结果MTM与MSM牙长轴夹角与MSMDBL呈正相关关系,当角度大于22.5°时,MSMDBL明显加重;当MTMMCEJ-MSMDCEJ距离大于6mm,MSMDBL明显加重,在9~12mm时最严重。结论可将近中阻生的MTM与MSM牙长轴夹角超过22.5°或者MTMMCEJ-MSMDCEJ超过6mm视为预防性拔除MTM的指导原则。Objective To discuss the relationship between the mesial impacted mandibular third molar and distal alveolar bone loss of the mandibular second molar.Methods Collected 274 cases’CBCT of mesial impacted MTM to measure the angle of long axis between MTM and MSM,the distance between mesial CEJ of MTM and distal CEJ of MSM(MTMMCEJ-MSMDCEJ),MSMDBL,and count the age,gender.The relationship and the influence of each factor on MSMDBL were analyzed.Results The angle of long axis between mesial impacted MTM and MSM was positively correlated with MSMDBL.When the angle>22.5°,MSMDBL was significantly increased.MTMMCEJ-MSMDCEJ was a strong positive correlation factor affecting MSMDBL.When MTMMCEJ-MSMDCEJ>9 mm,MSMDBL was the most severe.Conclusion The angle of long axis between the MTM and MSM>22.5°or the MTMMCEJ-MSMDCEJ>6 mm could be regarded as the guiding principle for the preventive removal of MTM.
