乳腺非特殊性浸润性癌X线影像特征与病理分析

摘　　要：目的探讨乳腺非特殊浸润性癌(nonspecific invasive breast carcinoma,NSIBC)各组织类型的X线征象,分析其与病理组织学类型的相关性。方法回顾分析经手术病理证实的乳腺非特殊性浸润性癌患者200例,均做乳腺钼(或铑)双靶全数字X线检查,研究病变X线影像特征,以及其与肿瘤组织类型之间相关性。结果肿瘤X线表现为肿块165例,毛刺征103例,钙化80例,结构扭曲、紊乱27例;这些征象大部分为2个及2个以上同时出现。肿块样表现见于浸润性导管癌93例,单纯癌53例,髓样癌10例,黏液腺癌4例,混合癌3例。肿块并有钙化者67例。毛刺征见于浸润性导管癌67例和单纯癌32例,小叶癌3例,混合癌1例。结构扭曲、紊乱见于浸润性小叶癌13例,浸润性导管癌11例,单纯癌2例,混合癌1例,其中2例浸润性小叶癌和7例浸润性导管癌并钙化。病例仅单独钙化表现的4例。结论乳腺非特殊性浸润性癌不同组织类型影像表现各有特点,病变组织类型与其影像表现具有相关性,对临床早期诊断、分型及临床治疗方法选择具有一定价值。Objective To investigate the X-ray signs of various tissue types of nonspecific invasive breast carcinoma(NSIBC),and analyze their correlation with pathological types.Methods Retrospectively analyze 200 cases with nonspecific invasive breast carcinoma confirmed by surgery and pathology,all of them were examined by molybdenum(or rhodium)double target full digital X-ray,to study the X-ray image characteristics of the lesions and the correlation between the X-ray image characteristics and tumor tissue types.Results Tumor X-ray showed that 165 cases had mass,103 cases had burr sign,80 cases had calcification,27 cases had structural distortion and disorder;Most of these signs were two or more.Mass-like manifestations were seen in 93 cases with invasive ductal carcinoma,53 cases with simple carcinoma,10 cases with medullary carcinoma,4 cases with mucinous adenocarcinoma,and 3 cases with mixed carcinoma.There were 67 cases with calcification in the mass.Burr signs were found in 67 cases with invasive ductal carcinoma,32 cases with simple carcinoma,3 cases with lobular carcinoma,and 1 case with mixed carcinoma.Structural distortion and disorder were found in 13 cases withinvasive lobular carcinoma,11 cases with invasive ductal carcinoma,2 cases with simple carcinoma and 1 case with mixed carcinoma,including 2 cases with invasive lobular carcinoma and 7 cases with invasive ductal carcinoma with calcification.Only 4 cases showed calcification.Conclusion The X-ray findings of different tissue types of nonspecific invasive breast carcinoma have their own characteristics,and the lesion tissue types are correlated with their image expressions,which has certain value for early clinical diagnosis,classification and clinical treatment methods.
