探讨彩色多普勒诊断仪产前诊断胎儿宫内窘迫的临床价值

摘　　要：目的:探究对产妇在产前行彩色多普勒诊断仪诊断对于判断胎儿是否存在宫内窘迫的临床价值。方法:现将2019年2月~2020年2月来本院就诊的78例产妇进行研究,所有产妇均行彩色多普勒诊断仪进行诊断,将其中的胎儿窘迫孕妇39例作为实验组,另外的正常妊娠产妇39例作对照组。通过使用彩色多普勒诊断仪对两组胎儿血流变化特点进行对比,并对诊断胎儿宫内窘迫的准确率进行评价。结果:对实验组和对照组胎儿包括Vd、Vs、PI、RI、S/D等在内的脐动脉血流变化指标和大脑中动脉血流变化指标进行数据比对分析发现,所有组间差异均具有显著差异(P<0.05);使用彩色多普勒诊断仪对实验组产妇诊断为胎儿宫内窘迫灵敏度高达94.87%(37/39),特异度为89.7%(35/39),准确率达91.14%(72/78)。结论:对产妇使用彩色多普勒诊断仪进行诊断,可通过对胎儿大脑中动脉和脐动脉血流动力学特征参数进行评价,对胎儿是否存在宫内窘迫进行判断,该方法操作简单,具有较高的安全性,重复性较高。Objective:To explore the clinical value of color Doppler diagnosis for prenatal diagnosis of fetal distress.Methods:From February 2019 to February 2020,78 cases of pregnant women who visited our hospital were studied.All the pregnant women were diagnosed with color Doppler diagnostic apparatus.Among them,39 cases of pregnant women with fetal distress were taken as the experimental group and the other 39 cases of pregnant women with normal pregnancy as the control group.The color Doppler diagnostic instrument was used to compare the characteristics of fetal blood flow changes between the two groups,and the accuracy of the diagnosis of fetal distress was evaluated.Results:Data comparison and analysis of umbilical artery blood flow change indexes and blood flow change indexes of middle cerebral artery including Vd,Vs,PI,RI,S/D in fetuses of the experimental group and the control group showed that there were significant differences among all groups(P<0.05).The sensitivity and specificity of the color Doppler diagnostic instrument were 94.87%(37/39),89.7%(35/39),and the accuracy was 91.14%(72/78).Conclusion:The use of color Doppler diagnostic instrument for diagnosis of parturient can be used to evaluate the blood flow mechanical characteristics of fetal middle cerebral artery and umbilical artery,and to judge whether the fetus has intrauterine distress.The method is simple to operate,has high safety and high repeatability.
