PAX8和SATB2在卵巢黏液性肿瘤中的诊断与鉴别诊断

摘　　要：目的探讨PAX8和SATB2在卵巢黏液性肿瘤中的诊断与鉴别诊断。方法收集46份原发性卵巢黏液性肿瘤与49份转移性消化道黏液腺癌组织,采用免疫组化SP法检测CK7、CK20、PAX8及SATB2的表达情况。结果CK7、PAX8在原发性卵巢黏液性肿瘤中的阳性率明显高于转移性消化道黏液腺癌,比较有统计学意义(P<0.05)。SATB2在原发性卵巢黏液性肿瘤中的阳性率明显低于转移性消化道黏液腺癌,比较有统计学意义(P<0.05)。原发性卵巢黏液性肿瘤中呈CK7(-)/CK20(-)与CK7(+)/CK20(+)的比例与转移性消化道黏液腺癌比较无统计学意义(P>0.05)。原发性卵巢黏液性肿瘤中呈CK7(+)/CK20(-)/PAX8(+)/SATB2(-)的比例明显高于转移性消化道黏液腺癌,比较有统计学意义(P<0.05)。CK7(+)/CK20(+)的敏感性为61.9%,特异性为70.4%;CK7(+)/CK20(-)/PAX8(+)/SATB2(-)的敏感性为73.2%,特异性为83.5%。结论PAX8、SATB2有助于鉴别诊断卵巢黏液性肿瘤的来源,联合检测可明显提高敏感性与特异性。Objective To explore the diagnosis and differential diagnosis of PAX8 and SATB2 in ovarian mucinous tumors.Methods A total of 46 cases of primary ovarian mucinous tumors and 49 cases of metastatic digestive tract mucinous adenocarcinoma were collected and the expressions of 7,CK20,PAX8 and SATB2 were detected by immunohistochemical SP method.Results The positive rates of CK7 and PAX8 in primary ovarian mucinous tumors were significantly higher than those in metastatic gastrointestinal mucinous adenocarcinoma(P<0.05).The positive rate of SATB2 in primary ovarian mucinous tumors was significantly lower than that in metastatic gastrointestinal mucinous adenocarcinoma(P<0.05).The proportion of CK7(-)/CK20(-)and CK7(+)/CK20(+)in primary ovarian mucinous tumors was not statistically significant compared with that of metastatic gastrointestinal mucinous adenocarcinoma(P>0.05).The proportion of CK7(+)/CK20(-)/PAX8(+)/SATB2(-)in primary ovarian mucinous tumors was significantly higher than that in metastatic gastrointestinal mucinous adenocarcinoma(P<0.05).The sensitivity and specificity of CK7(+)/CK20(+)were 61.9%and 77.4%respectively.The sensitivity and specificity of CK7(+)/CK20(-)/PAX8(+)/SATB2(-)were 73.2%and 83.5%respectively.Conclusion PAX8 and SATB2 can help to differentiate and diagnose the source of ovarian mucinous tumors,and the combined detection can obviously improve the sensitivity.
