血清外泌体长链非编码RNA RN7SK在胃癌中的诊断价值

摘　　要：目的:检测胃癌患者血清外泌体长链非编码RNA RN7SK(lncRNA RN7SK)表达水平,探讨其对胃癌的诊断价值。方法:收集2016年12月至2017年12月山东大学齐鲁医院就诊的胃癌患者(116例)、不典型增生患者(43例)、慢性胃炎患者(48例)及健康对照受试者(36例)血清,提取外泌体。q RT-PCR检测lncRNA RN7SK的表达,分析其与胃癌临床病理学特征的关系,电化学发光法检测血清CEA水平,绘制ROC曲线,分析其作为胃癌新型标志物的诊断价值。结果:血清外泌体lncRNA RN7SK在胃癌患者的表达[2.175(1.475,3.223)]高于不典型增生患者[1.300(0.980,1.610)]、慢性胃炎患者[1.030(0.560,1.358)]及健康对照受试者[1.005(0.650,1.478)](P值均<0.001);不典型增生患者高于慢性胃炎患者(P<0.001)及健康对照受试者(P=0.037)。血清外泌体lncRNA RN7SK表达水平与胃癌患者肿瘤直径、分化程度、淋巴结转移、远处转移及TNM分期显著相关,与患者性别、年龄、Bormann分型无关。外泌体lncRNA RN7SK的ROC曲线下面积(AUCROC)为0.84(95%CI:0.772~0.895),特异性和敏感性分别为88.9%、66.4%,其检验效能优于CEA 0.580(95%CI:0.497~0.659)。二者联合ROC曲线下面积(AUCROC)为0.854(95%CI:0.788~0.906),特异性和敏感性分别为83.3%、75.9%,优于lnc RNA RN7SK和CEA。结论:lncRNA RN7SK在胃癌患者血清外泌体中高表达,与肿瘤直径、浸润深度、淋巴结转移、远处转移、TNM分期具有高度相关性,且具有良好的诊断价值,可作为潜在的胃癌诊断分子标志物。Objective:To explore the expression of exosomal long non-coding RNA RN7 SK(lncRNA RN7 SK)in serum of patients with gastric cancer(GC)and its screening value in clinical diagnosis of GC.Methods:The expression of exosomal lncRNA RN7 SK mRNA in serum of patients with GC(116 cases),atypical hyperplasia(43 cases),chronic gastritis(48 cases)and healthy controls(36 cases)was detected using real-time quantitative RT-PCR.Serum CEA was also measured by electrochemiluminescence assay.The screening efficacy was detected by the receiver operating characteristic(ROC)curve.Results:Serum exosomal lncRNA RN7 SK was found to be higher in patients with GC([2.175(1.475,3.223)])compared with atypical hyperplasia[1.300(0.980,1.610)],chronic gastritis[1.030(0.560,1.358)]and healthy controls[1.005(0.650,1.478)](all P<0.01).The expression of lncRNA RN7 SK in atypical hyperplasia was higher than chronic gastritis and healthy controls.Serum exosomal lncRNA RN7 SK was significantly correlated with tumor diameter,differentiation,lymphatic metastasis,distal metastasis and TNM stage.The AUC of exosomal lncRNA RN7 SK was 0.84(95%CI:0.772,0.895),with 66.4%sensitivity and 88.9%specificity,and was higher than that of CEA0.580(95%CI:0.497,0.659).The combination of 2 markers gave an AUC of 0.854(95%CI:0.788,0.906),which was better than with the individual marker.Conclusion:The level of serum exosomal lncRNA RN7 SK is significantly overexpressed in GC patients and correlated with tumor diameter,differentiation,invasion depth,lymphatic metastasis,distal metastasis,and TNM stage so that it may be a potential biomarker for GC diagnosis.
