中医穴位按摩干预儿童青少年调节性和轻度近视的meta分析

摘　　要：目的采用系统评价方法对中医穴位按摩治疗儿童青少年调节性和轻度近视的临床疗效。方法计算机检索知网(CNKI)、万方数据库、维普数据库(VIP)、中国生物医学文献服务系统(CBM)和pubmed英文数据库查阅与中医穴位按摩治疗儿童青少年近视相关的RCT文献。严格筛选纳入文章对其治疗近视的有效率进行meta分析(RevMan 5.3软件和stata 16.0),采用Cochrane手册提供的偏倚风险评价工具评判纳入文献随机对照试验的质量,通过森林图和漏斗图进行偏倚性分析。结果纳入符合实验标准的9篇RCT文献,共873位1745眼6~20岁患者(实验组928眼;照组817眼)。Meta分析结果显示:中医穴位按摩对于儿童青少年调节性和轻度近视疗效显著,有效率优于睫状肌麻痹剂或纯中药治疗,实验组与对照组差异具有统计学意义(RR=1.32;95%CI[1.26,1.36],且统计学意义显著Z=11.15,P<0.00001)。结论中医穴位按摩干预儿童青少年调节性和轻度近视疗效优于睫状肌麻痹剂或纯中药等药物治疗,未来还需更高质量RCT研究进一步论证。Objective To evaluate the clinical efficacy of traditional Chinese medicine acupoint massage in the treatment of accommodative and mild myopia in children and adolescents.Methods(CNKI),Wanfang database,VIP database(VIP),Chinese biomedical literature service system(CBM)and pubmed English database were searched for RCT literatures related to acupoint massage in the treatment of myopia in children and adolescents.The effective rate of myopia was analyzed by meta(RevMan 5.3 software and stata 16.0).The quality of randomized controlled trials was evaluated by the bias risk assessment tool provided by Cochrane manual,and the bias analysis was carried out by forest map and funnel map.Results Nine RCT literatures were included,including 1745 eyes of patients aged from 6 to 20 years old(experimental group,928eyes;control group,817eyes).The results of Meta analysis showed that TCM acupoint massage had a significant effect on accommodative and mild myopia in children and adolescents,and the effective rate was better than that of ciliary paralysis or pure traditional Chinese medicine.there was significant difference between the experimental group and the control group(RR=1.32).95%CI[1.26 and 1.36],and the difference was statistically significant(P<0.00001).Conclusion The curative effect of TCM acupoint massage in the treatment of accommodative and mild myopia in children and adolescents is better than that of ciliary paralysis or pure traditional Chinese medicine.Higher quality RCT research is needed in the future.
